JOB DESCRIPTION

JOB TITLE: Social Services Coordinator

DEPARTMENT: ICF/MR

SUPERVISOR: Administrator

JOB SUMMARY

Serve as a liaison between the facility, other agencies, and parents/guardians. Ensure facility compliance with rules/regulations relating to informed consent.

MAIN DUTIES

I. Member Protection:

A. Ensure that members are not subjected to physical, verbal, sexual, or psychological abuse or punishment through staff.

B. Through direct contact, member activities and formal staff training, ensure the rights of all members (informed of facility rules, risks of treatment, due process, financial affairs to the extent possible, free from unnecessary drugs and restraint, association –communication, etc…).

C. Promote participation of parents/guardians in the process of providing active treatment (answering questions, promoting visits, notification of significant events or changes in conditions, etc…).

D. Assure that personal funds are accounted for and available upon request.

E. Ensure adult members have legal guardians with proper documentation provided to maintain guardianships.

Facility Staffing:

F. Ensure that staff are provided initial and continuing training that enables them to perform duties effectively, efficiently, and competently.

G. Serve as a member of the IDT providing direction, motivation, feedback, and encouragement to all staff involved with members.

Active Treatment:

H. Ensure that all member interactions and activities are age appropriate.

I. Ensure appropriate participation of agency and non-agency staff in the interdisciplinary team process to the extent appropriate.

J. Insure that the IDT is provided a preliminary evaluation that contains adequate background and other assessments prior to any decision on admission.

K. Insure that discharges/transfers are for good cause and members/parents/guardians are given reasonable preparation time.

L. Insure that at the time of discharge, the IDT develops an appropriate final summary and post-discharge plan that will assist in the members adjustment.

M. Insure that within (30) thirty days of an admission, the IDT meets to develop and individual program plan and then meets at least yearly thereafter to update the plan.

N. Insure that copies of the individual program plans are made available to all relevant staff, other agencies, and parents/guardians.

O. Insure that, 1). A specially constituted committee (Human Rights) is designated and used, 2) is made up of facility staff, guardians, and other persons with no ownership or controlling interest in the facility, and 3) the committee reviews, approves, and monitors programs that may involve risks to member protection and rights.

P. Insure that programs that may involve risks to member protection and rights are conducted only with the written informed consent of the member, parent, or legal guardian.

Member Behavior and Facility Practices:

Q. Ensure that staff demonstrate and use only facility approved interventions in the management of member behavior.

R. Insure that programs specified (per policy) are conducted only with the informed consent of the parent/guardian.

S. Insure that before implementation of more intrusive programs, 1) the member is allowed due process, 2) less restrictive methods are shown to be ineffective or contraindicated, 3) the procedures are not used for discipline or convenience, and 4) that the procedures will be used as part of an active treatment program.

T. Insure that all programs are in accordance with agency policy, federal, and state regulations. Insure that behavior modifying drugs, 1) do not interfere with daily living, 2) are used only as part of the members overall program, 3) are monitored closely, and 4) are gradually withdrawn at least annually unless contraindicated.

U. Insure that the facility has policies/procedures for the management of conduct between staff and members. That these procedures are current and available to staff, members, parents, and guardians.

Physical Environment/Health:

V. Ensure members environment is furnished and maintained in good repair.

W. Ensure that staff are familiar with the use of the facilities fire protection features and emergency plans/procedures.

X. Ensure that members and staff are provided a sanitary environment through proper infection control procedures.

Y. Ensure member cleanliness and proper appearance.

Z. Ensure members are treated with respect and dignity.

Training

AA. Participate in all training that enables performance of duties effectively, efficiently, and competently (e.g. – staff meetings, orientation, in-services, etc.).

AB. Demonstrate the skills and techniques necessary to administer interventions to manage the inappropriate behavior of members (proper client management, no over use or reliance on reprimands and/or negative statements).

AC. Demonstrate the skills necessary to implement the individual program plans for each member.

General Process:

AD. Maintain personnel records and ensure confidentiality.

AE. Develop and implement company policies/procedures as specified.

AF. Maintain lines of communication and foster effective working relationships between all employees and departments.

AG. Develop, maintain, and foster positive training environment between staff and members and staff and supervising authorities.

AH. Comes to work as scheduled and consistently demonstrates dependability and punctuality; complies with attendance policy.

AI. Participate in other activities assigned or dictated.

G. Document member contacts (e.g., family visits) and involvement

by family or significant others.

H. Maintain financial records for member funds including cash dispersements for activities.

I. Maintain records for billing of services.

J. Insure that proper contacts, agreements, and procedures are followed for payment of services provided by the facility.

K. Provide feedback and other information on members to families, social service departments, or other agencies as required.

L. Maintain updated admission waiting list.

QUALIFICATIONS

2. Must have a Bachelor’s Degree in the Human Services field.

3. Must have a valid driver’s license and meet insurance requirements for driving.

4. Must have or commit to obtain chauffeurs license, if required by facility.

5. Must be approved to work by the Iowa Department of Human Services if one or more of the following exist: prior convictions, child abuse registry report, and/or dependent adult abuse registry report.

6. Must not be on Medicare/Medicaid fraud registry.

7. Must pass pre-employment drug test.

8. Individuals with blood borne pathogen diseases cannot be employed in any direct care position due to risk of disease exposure to our members.
