The information can be collected by direct observation of the child during daily activities and play, structured eliciting situations and parent interviews. The information obtained can yield information about a variety of skills and behaviors in preschool children.OverviewDavid is a 3 year 7 month old boy who attends Wonder Years Preschool from Monday through Friday. He is currently receiving 10 hours of SEIT services. His classroom consists of 18 students, a classroom teacher and one assistant. David also receives 5x30 of Speech and 3x30 of Occupational therapy services. David is allergic to Tree nuts and Nature.CognitionDavid presents many pre-academic strengths. During individual one-on-one sessions with the SEIT, David is able to identify all uppercase letters of the alphabet. He also is beginning to associate letters with sounds. For example, when presented with the letter E, he will say “E begins with Edward. “ David is in the beginning stages of receptively identifying lowercase letters. He is able to match and sort objects by color, shape and size. In the mathematical domain, David is able to count from 1-19 without error. At times, he will rush and skip a number, but is easily redirected and will self-correct. He is fond of the number “19” and will ask the SEIT to count to this number randomly. David can count illustrating 1:1 correspondence from 1-15. He is able to identify all geometric shapes such as a triangle, square and circle. David is able to join his peers for large group time. He has improved in his ability to sit quietly and wait for his turn. However, during singing of songs or music and movement, he will sometimes join the group. Other times, he will sit on the rug and observe his peers. He is encouraged by the classroom teacher and SEIT to join in, but some sensory sensitivities have been observed. Modifications: Teacher modeling, repetition, visual cues.Age Range: scattered skills 3.2-3.7Speech and LanguageDavid presents with expressive articulation delays. He will often present substitutions or omissions of words at the single word level. The SEIT has been working collaboratively with the speech therapists in order to address these skills. Since the start of services, David has acquired more vocabulary terminology which helps in his ability to initiate conversation with teachers and peers. However, as his length of utterances increase, his speech intelligibility is compromised. At times, David will become frustrated when he is not understood and will give up. The SEIT is helping David to use other modalities in order to help his message to be understood. For example, pointing and/or gesturing.During verbal concepts, David is able to demonstrate an understanding of big/little, in, on and open/close. He is able to follow 2-step verbal directions. David’s ability to retell a story is emerging. His attention to story time is limited, depending on the book. If the book is about trains or other vehicles, he is able to sustain more interest. Modifications: RepetitionAge Range: scattered 3.0-3.6Social/Emotional SkillsDavid is an extremely sweet child that has formed friendships with certain children in his classroom. He has improved in his ability to use his words to join in play. He is able to demonstrate pro-social skills such as sharing, waiting for his turn and playing simple games. He is warmly responsive to his classroom teachers, but at times will have difficulty leaving the classroom to work with one of his speech therapists. David is able to transition into the different parts of the day. However, changes in the daily schedule can be challenging. If there is a change, the SEIT will prepare David for the anticipated change. Also, during a trip to the other school building, David became a bit anxious because he was told he would be going to a new school. The SEIT will work on this by creating a social story in regard to going to the “big school.” Overall, David is engaging more in associative and imaginative play.Modifications: Peer and adult modelingAge Range: 3.4-3.8Fine Motor:David has begun to use his left hand more frequently during fine motor tasks. He previously would avoid all fine motor tasks that involved painting or using glue. However, he will now use liquid glue, glue sticks or allow his hand to be painted. When tracing the letters in his name, David uses very faint strokes. During these pre-writing tasks, focus is spent on helping David develop proper form while holding the writing implement. He is not interested in drawing, but will mainly scribble on a page. He will color a picture, but not within the lines. He is not able to accurately draw shapes such as a circle.David does enjoy manipulating play dough and completing interlocking puzzles. He has strong visual perceptual skills and this helps him in completing puzzles. He also enjoys building with Lego’s.Modifications: Hand over hand assistanceAge Range: scattered 3.0-3.6Gross Motor:David is able to run, jump and climb. During outdoor play, he will try to keep up with his peers as they go up and down the slide. He is able to ride a tricycle, but needs reminders to avoid obstacles in his path. David is still learning to kick, throw and catch a ball. During music and movement time, David will try to participate. However, if the movements are new or too challenging, he will sit on the rug and observe. He is been observed to stand on one foot, hop and balance on his dominant leg.Modifications: Teacher modeling and encouragementAge Range: 2.8-3.8Adaptive/Self-Help Skills:David is still in the process of being toilet trained. Recently, he comes to school wearing underwear rather than a pull-up. Although he has had accidents, he is encouraged daily by the classroom teachers and SEIT to sit on the toilet. A visual chart will be implemented to help aid in this process.David is able to feed himself using a spoon. Especially when eating yogurt, he will use a spoon. There is some spilling, but he is praised for his efforts. He likes to be independent when it comes to snack time and will tell the teacher what he prefers to eat. Modifications: Teacher modeling; visual chartAge Range: 3.0-3.8SummaryDavid is a 3 year 7 month old boy who has shown improvement across all areas of his development. He is acquiring more speech and language skills which foster growth in his social emotional development. However, there are still areas of concern. It is recommended that David continue receiving his current menu of services in order to maintain his gains.
