PROGRAM BASICS

The Weston Group, Inc. presents private pay programming designed to guide the client through an exercise regime geared specifically to his/her needs.

ADVANTAGES to participation are:

- Scheduled based on the client’s desire

and tolerance level, which is jointly

determined by the therapist, the client,

PRIVATE SERVICES

Following skilled services or a therapy evaluation:

• The therapist, in conjunction with the client, will determine what the client's needs are and if the PEP! Program is appropriate to meet those needs

• If non-skilled services are indicated, the therapist will obtain orders from the physician to discharge the client from skilled services, and to begin non-skilled exercise sessions

• The therapist will update orders monthly to continue participation in the non-skilled session for as long as the client desires

• The program is open to everyone, even those who have not been on skilled therapy; however, a thorough evaluation will be completed by a Licensed Therapist prior to acceptance in the PEP! Program

Dining Needs

and the Elderly

Difficulty swallowing and/or choking, termed dysphagia, is one of the most common conditions among the elderly. In fact, as much as 65% of the elderly population in skilled nursing and long term care facilities are at risk for choking. Although the occurrence of dysphagia may be less in assisted living and retirement communities, the consequences of dysphagia are just as significant.

In addition to swallowing disturbances, residents often experience difficulty with the actual process of feeding. Upper extremity contractures, decreased posture, decreased coordination and strength, and /or arthritic changes can all reduce an individual’s independence with the act of eating.

Signs and Symptoms of Dysphagia

• Repeated Swallowing

• Chronic Coughing

• Frequent Clearing of throat and husky voice

• Escape of food or saliva from mouth during eating

• Unexplained loss of weight

• Pocketing of food in mouth

• Weight loss ( 5lb. In 30 days)

• Pain with swallowing

• Recurrent Lower Lobe Pneumonia

Food or liquid can also be “silently” aspirated, meaning that the substance is entering the lungs without any external signs or symptoms.

Goals of the Dining Program

• Reduce risk of aspiration/improve swallowing

• Increase independence with feeding

• Improve nutritional intake and hydration

• Increase socialization

• Improve self esteem

• Educate caregivers concerning safe and appropriate eating

• Contribute information necessary to alter diet consistencies or food types

• Determine appropriate adaptive equipment

The Purpose of the Dining Program

Eating habits are truly individualized and problems may occur due to various interacting factors such as poor oral health, medications, decreased strength, and/or difficulty swallowing. These problems have significant implications not only for health and safety but also for socialization and general well being. Independence in the dining room helps preserve dignity and the enjoyment of socializing with others, while ensuring nutritional needs are met. The Weston Group, Inc. presents programming fro those individuals with dining needs.

Components of the Dining Program

• Dining Assessment Screening

• Skilled Occupational and/or Speech Therapy

• Facility In-services and Educational Training sessions for residents and families

• Diet/Food recommendations

• Adaptive Equipment recommendations, attainment and training

Licensed Occupational and Speech Therapists and Assistants are responsible for the implementation and general oversight of the Independence with Dining Program.

Modifications for Dysphagia

Physical Modification

• Sit up straight while eating, with shoulders level

• If muscles in your jaw and tongue are weak, you can be taught exercises to increase strength and making chewing more effective

• If you have false teeth, make sure they are properly fit

Swallow Modification

• Be sure your food is cut in small enough pieces

• Allow plenty of time; do not rush through a meal

• Try to eat in a peaceful setting

Food Modification

• Liquids should be thickened (thickening agents are available) in order to control the flow

• Solid foods should be softened or pureed, requiring less chewing and propulsive force to clear them through the pharynx

• Food must look good to stimulate appetite. Puree meat and vegetables separately so they look appetizing

LOW VISION TIPS

1. Magnifiers

Magnifiers come in a variety of magnification strengths and styles. They can be used to increase the size of written print.

2. Lighting

Lighting should be bright, but without glare, and should be consistent throughout the house. Use 100 watt bulbs in overhead fixtures.

3. Use of Contrast

Use contrasting colors to make objects more visible. For example, a white plate on a black tablecloth, or black coffee in a white mug.

4. Safety

Remove electrical cords from pathways, be sure rugs are tacked down, and keep rooms spacious and free of clutter.

5. Telephones

Use phones with extra large or illuminated keypads.

THE INS AND OUTS OF LOW VISION

Low Vision: A reduction in vision that cannot be corrected with glasses, contact lenses, medical intervention, or surgery. The loss of sight is not total, which would be blindness, but rather the individual is left with some usable vision.

- Affects individuals of all ages.

- Diagnosed: 14 million Americans

- Prevalence: Affected individuals expected to double by year 2030

- Causes: Injury, increasing age, or a disorder

- Primary Causes: Macular Degeneration and Diabetes

- Symptoms: Difficulty recognizing faces, difficulty reading (text appears broken or distorted), difficulty seeing objects or obstacles, increased light sensitivity, night blindness, color confusion, and blurred vision.

REHABILITATION SERVICES

Occupational and Physical Therapy services can greatly impact the level of independence for an individual with low vision. Our goal is to maximize the individual’s ability to function safely, independently, and confidently with low vision.

- Adaptive equipment (talking watch, magnifier, large print books, etc.)

-ADL’s - bathing, dressing, home management, finance management

-Gross and fine motor activities

-Education in compensatory techniques

How Can a Physical Therapist Help You?

The ability to move is not merely a matter of using your limbs or handle objects. There can be cardiac or pulmonary problems that may interfere with your body’s ability to use oxygen to fuel muscles for movement. Physical Therapists work with patients to improve mobility, thus increasing the benefits of oxygen flow.

Physical Therapists do not only treat you when you are ill or injured. They can develop a program geared toward prevention and wellness.

Some ways that you can help maintain your heart’s health:

• Avoid exposure to tobacco smoke

How can an Occupational Therapist help your heart health?

A person with cardiovascular disease may have many questions about how to continue with Activities of Daily Living (ADLs). They may wonder how they can engage in the activities they have always enjoyed without jeopardizing their heart’s health.

The Weston Group, Inc. Occupational Therapists can help you by:

• Evaluating your physical and psychological health

• Determining what activities need modification to reduce strain on the heart

• Helping you expend less energy by suggesting pacing strategies or adaptive equipment to reduce stress on your heart

• Working with you on scheduling tasks to minimize stress and fatigue

You can help yourself by implementing a progressive activity program to help build strength and endurance and by learning relaxation and stress management techniques.

When can a Speech Therapist

Assist you?

While a Speech Language Pathologist (SLP) does not play a major role in the prevention of heart disease, a person who has suffered a heart attack or stroke will find value in their role in Post-stroke rehabilitation.

A stroke or heart attack can have an effect on the victim’s speech, cognition, and facial muscle tone.

After a stroke, an individual may experience

• Aphasia (language impairment)

• Dysarthria (weakness of the muscles used in speech production)

• Dysphagia (swallowing disorder)

In the days, weeks, and months immediately after a stroke, an individual may experience “spontaneous recovery”. During this time, the effects of the episode may begin to diminish on their own.

The Weston Group, Inc. Speech Language Pathologists can help the affected individual to improve beyond what will occur naturally and teach compensatory techniques to overcome speech and language deficits as well as swallowing disorders.

The Importance of Physical Activity

Physical inactivity is a major risk factor for the development of Heart Disease.

Significant health benefits can result with even a moderate amount of physical activity.

The effects of chronic diseases can also be reduced by engaging in regular exercise.

Regular exercise will not only strengthen the heart muscle, it can also reduce high blood pressure and help lower cholesterol.

Engaging in a well rounded, medically appropriate exercise program allows you to become involved in the management of your own symptoms and diseases.

You do not need to exercise at high intensity to gain health benefits. Something as pleasurable as walking or gardening can bring rewards.

Aerobic exercise-anything that gets oxygen into your system and works your heart and lungs-should be the focus of your activities.

Consider these activities (but consult with your Physician first!):

• Walking

• Dancing

• Jogging

• Bicycling

Choosing an activity that you will enjoy will motivate you to get into a healthy routine.
