Proof of Eligibility

You can use one of the following items for proof of eligibility, as applicable. Be sure to write your Sprint wireless number on each page you fax to Sprint. ? Current company/university/organization identification ? Current membership card with your name and member ID

(Note: Membership card cannot be handwritten.)

? Unexpired Credit Union debit / credit card ? Recent pay or pension stub issued within the last 90 days ? Current statement of account issued within the last 90 days ? Veteran ID card, Department of Defense form 214, copy of

military orders, expired military ID or military W-2

(Note: The Department of Defense does not permit active military IDs to be photocopied, scanned or faxed as a proof document.)

Cover, delete or black out all sensitive information such as Social Security number, financial institution account numbers, credit card numbers and salary details. Write your Sprint phone number on every page of your documentation. Sprint is not liable for confidential information.

Business cards or letterhead are not acceptable items for showing eligibility.
