You can sign up and register for the Patient Portal by giving your provider your name and email address. In turn, your provider will give you an activation code, known as an enrollment token. The enrollment token will allow you to log in to the system and create your own private user name and password. Or, check your provider's website for an "enroll now" button, which will allow you to register yourself.

Who can sign up?

Patients and their legal guardians can sign up for Patient Portal in accordance with the Terms and Conditions of the website. Just tell your provider who you would like to enroll and you are on your way to connecting with your doctor.
