The important thing is that LH peaks are not the same in elderly people, such that LH does not stimulate ovulation in women. For clarification, see p. 192. Men also have changed LH pulsations. See p. 216 for details. This is the most important point.

What are FSH changes with aging?

FSH is more unclear--there has been shown to be a slight increase in FSH (see p. 192) in women with the start of menopause. I don't think we really discussed this with men.

Gonadotropins:

Change to pulsatile pattern:

increased "Monotropic FSH " (ie increased FSH)

1st Noticed prior to any change in cycle length

Ovarian Steroidal Hormones

Estrone levels up early in the cycle in older ovulatory women

Possible due to LH/FSH alterations

Eventually, H-P-G axis is unable to generate LH surge needed for ovulation

In the male, with Age:

On the average, the male reproductive function remains normal (or only slightly diminished in some individuals) until advanced old age (80+ years) when reproductive function decreases.

Subtle changes include decreases in the following:

GnRH

Sensitivity of androgen secretion to LH

Sensitivity of negative feedback between GnRH and LH

Also in men, with aging, loss of high-amplitude LH pulses despite normal or increased pituitary LH stores
