We are seeking to recruit a Consultant Orthopaedic Surgeon with a specialist interest in lower limb arthroplasty. The post is available to start from August 2018. The post holder will be expected to play a full part in running the established lower limb arthroplasty service but concentrating on hip and revision hip arthroplasty.

The hip team at Glasgow Royal Infirmary consists of 3 consultant orthopaedic surgeons (including this post), extended scope physiotherapists and specialist nurses and attracts complex tertiary referrals from around the region. There is a Senior Nurse Practitioner who runs the outpatient arthroplasty service following all hip and knee arthroplasty patients. Currently, we are running a pilot project looking at offering ‘open appointments’ for this group of patients rather than routinely following them up.

The post holder will be expected to participate in the general orthopaedic trauma on-call rota.

The orthopaedic unit at Glasgow Royal Infirmary prides itself on its research output with numerous publications in peer-reviewed journals each year, as well as many podium presentations and posters at national and international meetings. There is a full time research manager who facilitates research and audit in the department. Excellent facilities also exist for basic science research in collaboration with Glasgow University and the Bioengineering Department at Glasgow Caledonian and Strathclyde Universities.

Applicants must have full registration with the General Medical Council, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Trauma and Orthopaedic Surgery or eligibility for specialist registration (CESR) or be within six months of confirmed entry at the date of interview. Non UK applicants must demonstrate equivalent training and have some experience of working in the NHS.

Applicants should demonstrate a clear interest and competence in managing lower limb conditions, particularly hip arthroplasty and a specialist advanced fellowship in this area is desirable.

HOSPITAL AND CLINICS WHERE SERVICES ARE TO BE PROVIDED

Glasgow Royal Infirmary is a University teaching hospital located in the east of Glasgow. The Directorate of Orthopaedics (North East) is based at Glasgow Royal Infirmary. The population served is approximately 340,000.

There are currently 16 Substantive Consultant Orthopaedic Surgeons (including this post) and 1 Locum consultant in the Unit where the full range of trauma and elective orthopaedic sub-specialties is covered. It is anticipated that the West of Scotland major trauma network plans will be implemented starting in 2019 and it is likely that Glasgow Royal Infirmary will be designated as a Trauma Unit and the Queen Elizabeth University Hospital will be the Regional Major Trauma centre.

The orthopaedic wards at Glasgow Royal Infirmary are located in Queen Elizabeth Building in close proximity to the theatres and ITU and also in the Surgical Block. Inpatient facilities for both trauma and elective work are based here. There are 87 inpatient beds split between 4 wards at Glasgow Royal Infirmary, in addition there are three 23 hour stay beds at the day surgery facilities at Stobhill hospital.

There are three laminar flow theatres for Elective Orthopaedic work. There are two trauma theatres at Glasgow Royal Infirmary, one general trauma theatre and one sub-specialist trauma theatre. Both are located in the main theatre suite, with one available for the management of Orthopaedic and soft tissue trauma seven days a week and the other four days a week. 10 day surgery sessions for Orthopaedics (Monday to Friday) are available at the Ambulatory Care Hospital (ACH) on the Stobhill site where 23 hour stay facilities are also provided Monday to Thursday.

Elective Orthopaedic and Fracture clinics are based at Glasgow Royal Infirmary with a dedicated X-ray facility and medical records department. The processing of patients through the Outpatient department is greatly facilitated by extended role Specialist Orthopaedic Physiotherapists and Podiatrists who initially assess a high proportion of referrals from Primary care. A total of 38 sessions per week of Specialist Physiotherapist time is allocated to this task.

The highly successful fracture clinic redesign has resulted in a significant reduction in unnecessary attendances for simple fractures. Guidelines and protocols are agreed with the Emergency Department for many of the fractures and patients are only seen following a triage of their x-rays and Emergency Department assessment by consultants in a virtual clinic. This has allowed patients to be seen in sub-specialist fracture clinics.

A number of post-operative follow-up clinics are run by nurses, physiotherapists and podiatrists. There are nurse-led fracture clinics, hand and wrist fracture fixation follow-up clinics and ankle ORIF clinics. Currently, we are running a pilot project looking at redesigning the outpatient services with the help of Scottish Government funds. For selected conditions, patients are sent information leaflets and are asked to ‘opt-in’ to be seen face-to-face in the clinic for an appointment. Similarly, instead of routinely following the arthroplasty patients, they are discharged at 3 months and are given ‘open appointments’ and are encouraged to access the service as and when they need it.

HOSPITAL AND DEPARTMENTS

Glasgow Royal Infirmary is a major teaching hospital and has all major specialties on site with the exception of Neurosurgery and Cardiothoracic surgery. There is an excellent Intensive Care Unit and High Dependency Unit for postoperative patients who require invasive monitoring, both run by the Anaesthetic Department.

The Centre for Rheumatic Diseases is a professorial unit based at Glasgow Royal Infirmary; this serves the regional population of West of Scotland and beyond and enjoys a close relationship with the Orthopaedic Department. The Musculoskeletal regional oncology service for the West of Scotland is located at Glasgow Royal infirmary. As a result, the department has the largest workload dealing with bony metastatic disease of any orthopaedic unit in the country. There are also close ties with the Imaging Department with ready access to both CT and MRI scanning, including a dedicated extremity scanner and 2 consultant radiologists with musculoskeletal interest.

ACTIVITY

There were:

• approximately 16,000 new outpatient attendances in 2016/17 (including fracture clinics)

• approximately 3,833 elective procedures in 2016/17

• approximately 3,205 trauma admissions in 2016/17

TEACHING AND RESEARCH

The department has close links with the University of Strathclyde Department of Bio-Engineering and the National Centre for Prosthetics and Orthotics. Regular collaborative research is undertaken with the Universities of Glasgow and the Caledonian University

The Consultant members of the department are involved in Undergraduate teaching for the University of Glasgow. The Unit is also recognised for Basic Surgical Training and for Higher Surgical Training in Trauma and Orthopaedics. There is a postgraduate teaching programme for higher surgical trainees organised on a West of Scotland basis. Education is strongly supported within the unit with daily multi-disciplinary trauma meetings, regular educational meetings and a bi-monthly audit meeting.

In addition to the scheduled direct clinical care activities (DCC) above, there is also DCC allocated to cover administration related to patient care, ward rounds and virtual fracture clinics. These fixed and floating DCC PAs add up to 9 PAs in total. There is 1 SPA allocated for SPA. 

JOB PLAN

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director.  NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings.  The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

Once the candidate has been appointed more SPA time may be agreed for activities such as undergraduate and postgraduate medical training which takes place outside direct clinical care, as well as research and/or management.  These activities must be specifically and clearly identified and be agreed with the candidate and desired by the department.
