CalFresh Healthy Living Statewide TrainingTraining Request FormLocal Implementing Agency (LIA) Training Request FormAbout UsThe CalFresh Healthy Living Statewide Training services are brought to you on behalf of the California Department of Social Services and in collaboration with CalFresh Healthy Living State Implementing Agencies; namely, Catholic Charities of California, California Department on Aging, California Department of Public Health, and CalFresh Healthy Living, University of California.Three-Step Request ProcessWe are excited to provide you with a list of training options for your LIA staff and subcontractors. The three-step process for completing the form are:Review the criteria for completing the request,Complete the request form at least 6-8 weeks prior to the requested training date, andSubmit a copy of the request form and any questions you may have to CASNAP-EdTraining@cdph.Request CriteriaThe following criteria helps us make the most of training staff time and resources, and ensures the best training results:Ensure a minimum of 15 attendees, or a minimum 10 attendees for small, rural agencies,Partner with and open this training opportunity to other nearby CalFresh Healthy Living agencies, andSecure adequate and free/low-cost space and internet access to accommodate the number of staff to be trained.Note: A confirmation of the request will be sent via email to the primary LIA upon receipt.
