1. Qualifying Transportation Alternatives (TA) Program Activities:

Check the activity as outlined in Title 23 USC, Section 213(b), FHWA Transportation Alternatives Interim Guidance, 10/24/2012, that the proposed project will address.

Transportation Alternatives Program, (23 U.S.C. 101 (a), (29))

❑ Construction, planning, and design of facilities for pedestrians, bicyclists and other non-motorized including sidewalks, bicycle infrastructure, pedestrian and bicycle signals, traffic calming techniques, lighting and other safety-related infrastructure projects, and transportation projects to achieve compliance with the Americans with Disabilities Act of 1990.

❑ Conversion of abandon railroads corridors for trails

❑ Turnouts, overlooks and viewing areas

❑ Community Improvements including:

• Inventory, control, or removal of outdoor advertising

• Historic preservation and rehabilitation of historic transportation facilities

• Vegetation management – to improve roadway safety, prevent invasive species and provide erosion control

• Archaeological activities – related to impacts from implementation of a transportation project

❑ Environmental mitigation activity

• Stormwater management, control, pollution prevention, or abatement related to highway construction or runoff

• Reduce vehicle-caused wildlife mortality or restore and maintain connectivity between habitats

Recreational Trails Program (23 U.S.C. 206), (23 Sec. 213 applies to TA funded RTP projects)

The Recreational Trails Program is administered by DEP. Since Florida opted out of the set-aside. The funds are available for any eligibility categories listed above. It does not mean that FDOT has a separate Recreational Trails Program. . Any applications for those listed below should be sent to DEP.

❑ Equestrian activities, including carriage driving

❑ Bicycling or use of other human-powered vehicles

❑ Aquatic or water activities

❑ Motorized vehicular activities, including all-terrain vehicle riding, motorcycling, snowmobiling, use of off-road light trucks, or use of other off-road motorized vehicles.

Safe Routes to School Program (23 U.S.C. 402), (Guidelines for Safe Routes to School, 11/30/2012):

❑ To enable and encourage children to walk and bicycle to school

❑ To make bicycling and walking to school a safer and more appealing transportation alternative

❑ To facilitate projects and activities that will improve safety and reduce traffic, fuel consumption, and air pollution in the vicinity of schools.

❑ Natural Resource or public land agencies

❑ School districts, local education agencies, or schools

❑ Tribal governments

(e) Describe any proposed right of way acquisition, including expected matching fund source, limitations on fund use or availability, and who will acquire and retain ownership of proposed right of way.

(f) Describe any related project work phases that are already complete or currently underway.

(g) Describe how the public was engaged in the project selection process; and, how were Environmental Justice principles addressed during the planning and decision making process.

(h) Other specific project information that should be considered. (Including possible significant environmental issues)

3. Project Implementation Information:

Attach documentation as exhibits to this application.

(a) Describe the proposed method of performing (i.e. contract or in-house) and administering (i.e. local or state) each work phase of the project. If it is proposed that the project be administered by a governmental entity other than the Department of Transportation, document that this entity has the fiscal, managerial, environmental and engineering capabilities to manage the work phase of the project consistent with federal requirements. This is accomplished by the local entity being certified under the Local Agency Participation (LAP) Program rules.

(b) Describe any public (and private, if applicable) support of the proposed project. (Examples include: written endorsement, formal declaration, resolution, financial donations or other appropriate means).

(c) Describe the proposed ownership and maintenance responsibilities for the project when it is completed. (A maintenance agreement with FDOT is required.)

(d) Describe any source of matching funds and any restrictions on availability. (Since the program is soft-matched, the local government is not required to provide a match.)

CERTIFICATION OF PROJECT SPONSOR

I hereby certify that the proposed project herein described is supported by,

(municipal, county, state or federal agency, or tribal council) and that said entity will: (1) provide any required funding match; (2) enter into a maintenance agreement with the Florida Department of Transportation; (3) comply with the Federal Uniform Relocation Assistance and Acquisition Policies Act for any Right of Way actions required for the project, and (4) support other actions necessary to fully implement the proposed project, and (5) enter into a LAP Agreement with the Florida Department of Transportation. I further certify that the estimated costs included herein are reasonable and understand that significant increases in these costs could cause the project to be removed from the Florida Department of Transportation work program.
