DriverAdvisor( monitors drivers monthly for any new activity or change in license status. Monthly emails are sent and when new violation activity or change in license status is detected, an MVR is automatically ordered and charged to customer account. See Turnaround Schedule for availability.

DriverSafe( Plus is an analytic reporting system based on MVR information stored within DriverSafe Plus. Using MVR data, this platform provides focused risk management analysis using charts, graphs, and tables to highlight various driver risk factors. With DriverSafe Plus, MVRs are scored to provide objectivity and quick MVR review. Also, analysis of drivers monitored with DriverAdvisor is incorporated into the data. DriverSafe Plus pricing is as follows:

OTHER FEE INFORMATION:

The initial account setup fee is $75.00. This is a one-time charge. This fee is not assessed to clients updating paperwork.

In addition to the above fees, a single MONTHLY $7.50 access fee is charged in the months reports are ordered. No access fee is charged in months when the service is not used.

As an option, customers may receive additional supporting detail in the invoice for $5.00 per month. This detail shows each request, the date ordered, the requesting user, and comments that were attached to the request.

Below is the list of the background reports available for the states we discussed. Because the report costs vary by state and new reports are made available, the information here may be subject to change at any time. Pricing and product description (including turnaround) per report is listed online and confirmed prior to ordering.

We have two methods of ordering: Check Plus and Classic View.

Check Plus is a standardized trace based criminal history focused at the county level.

Classic View allows the user to pick and choose between compiled state and individual county reports.

Both programs utilize the address history from the subject’s Social Security Number (SSN) Verification. The SSN Verification comes standard with the Check Plus. However, the classic view makes recommendations based on the address history of the SSN. All reports are ordered and fulfill online.

|CheckPlus – Turnaround varies - $34.95 (plus any additional court fees) |

|CheckPlus is an automated criminal record search that triggers manual county level searches based on a person’s address history. CheckPlus analyzes the last |

|seven years of address history and automatically searches up to five counties where address history was most recently discovered. CheckPlus does not use |

|database information and only draws upon the most current information available at the county court level. This search offers a standardized procedure to apply|

|uniformity to hiring practices – each employee or applicant is screened under the same set of guidelines but the search is focused where that employee has |

|been. |

|This report may be supplemented by adding database reports: |

|Federal Criminal Report (normally $10.95) is $8.95 when ordered in conjunction with CheckPlus. |

|Nationwide Sex Offender Registry (normally $4.95) is $2.95 when ordered in conjunction with CheckPlus. |

|Adverse Action Notification $6.95 each |

|Adverse Action notification is obligatory when consumer reports are being utilized to screen for employment purposes. The purpose of the pre-adverse action |

|letter is to notify the consumer that an adverse employment action will be taken due to information found on a report that your company ordered through iiX. |

|The pre-notification gives the employee the ability to rectify any outstanding issues within a specific period of time. |

|We provide access to Pre-Adverse Action and final Adverse Action letters. The pre-adverse action package contains the letter, the copy of the report(s) |

|utilized in the decision and a summary of rights. The final Adverse Action notification is the letter notifying the prospective/existing employee that the |

|adverse action has taken place. We compile and mail all information to the prospective or existing employee. This is an easy way to automate what can be a |

|time-consuming process! |

| |

|Social Security Number Verification - Instant Cost: $3.00 |

|iiX recommends an SSN Verification and Address History report as the first step toward a thorough criminal history search. This report checks the validity of a|

|submitted SSN - including whether the number is valid, the date and state of issue, and identifies possible former residences associated with the applicant. |

|The address history results can then be used to identify exactly which criminal searches are needed. |

| |

|Federal Criminal Report (Same Day) $10.95 each – State must be specified |

|Federal Criminal Record Search provides a detailed report on criminal cases prosecuted at the federal district court level. Note: This search is name match |

|only. Entering a middle initial is essential to processing this search correctly. |

| |

|Nationwide Sex Offender Registry (Instant) $4.95 |

|The National Sex Offender Registry currently includes sex offender information from all states except Connecticut. It also includes information from the |

|District of Columbia. Results will vary according to state. Records are generally updated on a monthly basis. |

|NOTE: The Connecticut Sex Offender Registry is available as a separate product. |

|Single-County Criminal Searches |

|Single County Criminal Search is conducted for felonies and misdemeanors adjudicated at the Superior Court, or Circuit or District Court. The turnaround time |

|begins on the day following submission, if the request is entered after 8 AM Eastern, and does not include weekends or holidays. See information on single |

|county below. |

|State Criminal Searches |

|Compiled state reports may not be available in all states. We consider the CheckPlus (previous page) a best practices report in that it is a standardized |

|request but focuses the search on the subject’s address history. |

| |

|Educational Verification $12.00 each per educational institution |

|Education history is the most commonly falsified information on resumes and job applications. Education Verifications allow you to request confirmation of an |

|applicant's stated educational attainments. iiX representatives will attempt to verify with each institution submitted the school attended and the state where |

|the school is located, the type of school ( i.e. high school, college, etc.), the degree(s) earned, and dates of attendance. The report will also include the |

|name and position of the person at the institution who verified the information. Please note that iiX requires a release form be faxed prior to providing |

|information for this employee background check. Also, some institutions may outsource their verifications to a third-party organization which may charge an |

|additional fee for the service. In such cases, iiX will pass any such additional fee on to our customer. Average turnaround for verifications is 3 to 5 working|

|days, depending on the responsiveness of the institution. |

| |

|Employment Verification $12.00 each per employer |

|Employment Verifications are an important tool in conducting a thorough search of an applicant's background. Using information entered into a verification |

|request by you (the prospective employer), iiX will attempt to confirm information and details provided by the Applicant regarding their prior work history. |

|The service verifies dates of employment, job title, eligibility for rehire, salary, and includes the name and position of the person at the employer who |

|provided the verification information. |

|Our representatives will contact an applicant's previous employers and conduct telephone interviews, where possible. Be aware that some employers may outsource|

|this function to a human resources consulting company, which may charge a fee for the verification. In such cases, iiX will pass any such additional fee on to |

|our customer. Please note that iiX requires faxed signed consent from the applicant prior to providing information for this employee background check. |

| |

|DOT Compliant Employment Verification Reports (DOT) $16.50 each employer |

|This service provides a comprehensive DOT employment verification check in compliance with Section 391.23 of the Federal Motor Carrier Safety Regulations. The |

|supervisor questionnaire consists of a series of questions regarding past employment and previous alcohol and drug tests that are required for DOT employment. |

|The report verifies: dates of employment, position(s) held, salary, eligibility for rehire, and includes the name and position of the person who verified the |

|information. It also includes the employer's responses to questions regarding: accidents within the past 3 years, testing for controlled substance use, whether|

|the applicant had a blood alcohol test within the past 3 years that returned a BAC or 0.04 or greater, whether the individual refused a drug or alcohol test |

|within the past 3 years, and whether (to the employer's knowledge) the applicant has violated any other DOT drug or alcohol regulations. Any additional |

|comments provided by the previous employer are also included. iiX requires that a release form be faxed prior to providing information for this employee |

|background check. Also, some employers may outsource their verifications to a third-party organization which may charge a fee for the service. iiX will pass |

|any additional charges on to our customer. |
