Montgomery County Schools Form 1 10-11

Student Registration Procedure for Middle School Online Learning to include North Carolina Virtual Public School (NCVPS)

Students and Parents:

Students wishing to take online courses should carefully determine if they posses characteristics of successful online learners. A website from NCVPS asks questions of interested students. Montgomery County Schools (MCS) requires all potential students and their parents read this information . **If Internet service is unavailable, a hard copy will be provided.

Students interested in taking online courses must adhere to the following steps:

1) Complete the Pre-enrollment Online Application (FORM 2) and return to the E-Learning Advisor (ELA).

2) Student and parent sign this form, Student Registration Procedures for Online Learning (FORM 1), indicating that both have visited the NCVPS website and read “Is Online Learning Right for Me?”

3) Return this form to your ELA. The ELA and guidance counselor will begin the Eligibility Rubric. If qualified, the principal, teacher, and parent will each receive a questionnaire. These must be completed and returned to the E-Learning Coordinator for final approval. If you are not qualified, you will be notified by the ELA or guidance counselor.

4) ELA and guidance will work together to register you for online classes.
