Does the patient have a: Known allergy?
No Yes, and noted in patient chart
Difficult airway or aspiration risk? No Yes, and equipment/assistance available
Risk of >500 ml blood loss (7ml/kg in children)? No Yes, and blood cross matched and available
Before skin incision
(with nurse, anesthesiologist and surgeon)
Confirm all team members have introduced themselves by name and role.
Verbally confirm the patient's name, procedure, and where the incision will be made.
Has antibiotic prophylaxis been given within the last 60 minutes prior to incision?
Yes Not applicable
Anticipated Critical Events Has the protocol for malignant hyperthermia been reviewed?
To Surgeon: What are the critical or non-routine steps? What are the plans for use of throat packs? How long with the case take? What is the anticipated blood loss?
To Anesthesiologist: After review of patient history and exams, are there any patient specific concerns?
To Nursing Team: Has sterility of instruments (including indicator results) been confirmed? Are there equipment issues or concerns?
Is essential imaging displayed? Yes Not applicable
Before patient leaves operating room
(with nurse, anesthesiologist and surgeon)
Nurse Verbally Confirms (when applicable): The name of the procedure Sponge and needle counts are correct Throat pack(s) are removed Tongue suture is in place and secured Pain medication administered Specimen labelling (read specimen labels aloud, including patient name) Whether there are any equipment problems to be addressed
To Surgeon, Anesthesiologist and Nurse: What are the key concerns for recovery and management of this patient?
Patient Transfer Protocol To Surgeon:
Remain in the operating room complex until patient is safely transferred to recovery room
To Anesthesiologist and Nurse: Accompany the patient to the recovery room and review any intra-operative concerns and post-op management with staff there
