Welcome to the Sylvania School District! All new students (grades K-12) are processed through the Student Services Department located in the Administration Offices at 4747 N. Holland Sylvania Road. Office hours are Monday - Friday from 7:30 am - 4:00 pm.

In order to accommodate your registration quickly, we must comply with the legal requirements of the following:

Please help us by submitting all the necessary legal documents for the enrollment process. Incomplete documentation may cause delays in the entry of student to their assigned schools or possible exclusion/withdrawal from the District due to lack of verification of records per the above legal mandates.

The following documentation/information must be filed with Student Services: In complying with the legal requirements of the "Missing Child Act" and the Sylvania Schools Board of Education, the following information and documentation must be on file:

A parent may not enroll a new student without a withdrawal slip from the previous school. If you do not have the withdrawal slip, you will need to make another appointment to complete the registration.

Official Birth Certificate with raised seal (hospital certificate is not acceptable)- If you do not have an original, you may obtain a certified copy of a birth certification through the Toledo Lucas County Health Department - Vital Records Department at 635 N. Erie Street, Toledo, OH 43624. The fee is $20.00. They will make a certified copy for you while you wait, Mon.-Fri. 8:00-4:30 or call them at 419-2134100. You may obtain a certified copy of a birth certificate by mail by doing the following:

Custody Papers (if child is not living with both biological parents)- If legal custody has not been established, the petitioner must present a filed court motion for change of custody of the child. The petitioner has sixty (60) days from the time of enrollment to produce a Journal Entry of Judgment establishing custody proceedings.

The following pages need to be provided for any student whose parents are divorced:

3. Photo I.D. of Parent/Custodial Parent/Guardian must be presented when registering student. Also any paperwork showing a parental name change (i.e. divorce/re-marriage) must be presented.

5. Proof of Residency- You must provide any one of the following:

Current utility bill (showing your name and address) Signed Purchase Agreement (utility bill must be presented within 60 days) Signed Lease Agreement (utility bill must be presented within 30 days) Signed Building Contract (utility bill must be presented within 90 days)

If you are building a home in Sylvania, your child may attend Sylvania Schools for up to 90 days prior to your residence in the district. After 90 days, you will be charged tuition, which is approximately $43.40 a day per child.

For persons living with family or friends- You will need to have completed at the time of registration:

Section "C" (Statement of Domicile) needs to filled out and notarized Moving in with a person in an apartment will not be accepted, unless your name is

on the lease Current utility bill of the person you are living with Car registration, utility bill or government document with the parent name and same address as

the person you are living with (needed within 30 days)

6. Special Education -If your child is receiving Special Education services, you must bring a copy of the current IEP (Individualized Education Plan) and ETR (Evaluation Team Report) or MFE (Multi-Factor Evaluation). Child cannot start school until received.

7. Transcript/Withdraw Slip - Junior and Senior High School students must bring a copy of his/her transcript and a withdraw slip from the previous school attended.
