I.
GENERAL STATEMENT
Houston Methodist provides College Scholarships for qualified employee's children who enroll in approved programs leading to a degree in a health care profession.
II. PROCEDURE
A. Eligibility
For a Houston Methodist employee's child to be eligible the employee (parent/guardian) must be actively employed with weekly standard hours of 20 or more and not be classified as temporary or PRN. The employee must also be in good standing, and have worked for Houston Methodist for a minimum of 2 years of continuous employment in a benefit eligible employee status.
The child must be the legal dependent of the Houston Methodist employee and eligible up to age 26. The child must have graduated from high school with a minimum of a 3.0 on a 4.0 scale or have a current college GPA of 3.0 on a 4.0 scale (if less than 12 college credit hours must also provide high school transcript). The child must be accepted into an accredited undergraduate college or university program leading to an undergraduate degree in a health care profession identified by Houston Methodist as a shortage area. (See Appendix A ? Approved Positions). Masters and Doctorate level degrees are not eligible for the program.

Only one college scholarship can be awarded per employee's child per year.
All Scholarship applicants (first time and previously approved applicants) are required to complete the google form.

Applications will be accepted by HR Hub from March 1 to April 30
and presented to the Committee for review. Scholarships will be
awarded annually on or before June 15.
First time applicants and previously approved applicants must complete the google form that will be retrieved by HR Hub and presented to the Committee. All applicants must submit documents to college.scholardocs@ by April 30.

High school transcript as of the previous fall semester or college transcript as of the previous fall semester.
College acceptance documentation. Typed essay explaining why student is choosing medical profession. Typed resume that summarizes extracurricular, personal volunteer and
work activities. Previously, approved applicants must complete the google form and provide the following documents via email to college.scholardocs@ by the April 30.
Provide official Transcript
HR Hub will verify eligibility, enrollment status, academic standing and employee standing. HR Hub will submit all eligible applications to the Committee for approval. In addition to reviewing applications from current scholarship recipients, a maximum of 25 new scholarships will be approved each year.
Scholarship awards will be made at the sole discretion of the Committee. The Committee reserves the right to request additional documentation. Decisions by the Committee are final.
C. Approval Process
All awards are based upon the employee's benefit eligibility as follows: $1,000 for Benefit Eligible employees with Standard Hours of 32 or greater Standard Hours. $500 for Benefits Eligible employees with Standard Hours between 20? 31.
Recipients will be selected on the following criteria:
1. Proof of eligibility. 2. Other information submitted as part of the application process. 3. Any other criteria selected at the discretion of the Committee.
Once approved, the employee may reapply for the scholarship for up to three Page 2 of 4
years (Maximum of four scholarships).
D. Tax Treatment
The scholarship will be taxable income to the employee in the year in which the scholarship is given.

The "Committee" reserves the right to change this list at any time and may approve positions not listed if the "Committee" determines the position to be beneficial to the organization.
