FOR THE TENNESSEE DEPARTMENT OF EDUCATION
This step is for certified, licensed employees only. Note: email will not be changed until your TN teacher license has been updated.
? All certified, licensed employees must complete a name change application on TNCompass for their teaching license to be maintained accurately.
? Log into your TNCompass account or create and account and follow the instructions to edit your personal information.
? You must attach a legal document (marriage certificate/divorce decree, etc.) to complete this process with the state.
FOR RUTHERFORD COUNTY SCHOOLS
? Submit the Name Change form and attach: a. A new W-4(available from your bookkeeper or the payroll dept.) b. A copy of your new social security card c. A screen shot of your name change request in TNCompass (certified employees only)
Scan all documents and email to HR@ or courier to the Human Resources department.
**Once the name change is processed by Payroll your insurance information will automatically change.
FOR THE STATE OF TENNESSEE PENSION PLAN
? The Tennessee Consolidated Retirement System must be notified of name changes. TCRS Member Self-Service includes:
? Update your TCRS contact information ? View your entire TCRS account history ? Retrieve your member annual statement ? Maintain your TCRS beneficiary information
o For assistance with Member Self-Service, navigate to and access the Member Self-Service User Guide or demonstration video available under the Concord tab. You may also call 800-770-8277, option 8 with questions.
To complete the Name Change process you must submit this form along with:
? a screen shot of your name change application from TNCompass (certified employees only)
o Your email address will not be changed to reflect your new name until your application in TNCompass has been processed by the state of Tennessee and reflects your new name.
? a new W-4 reflecting name change
? a copy of your new Social Security card reflecting name change
