PERSONAL INCOME TAX NOTICE FACTS

*DOCUMENTATION REQUIRED FOR OPTION #1

Any of the following may be used to validate residency:

• Lease agreement(s)/mortgage statement(s) for the calendar year(s) in question

• Utility bills for the first and last month during the calendar year(s) in question

• Cell phone bills for the first and last month for the calendar year(s) in question

• A letter on company letterhead or notarized statement from your landlord substantiating the beginning and ending date of residency

**ADDITIONAL INFORMATION FOR OPTION 2 THAT MAY REQUIRE YOU TO FILE AN OHIO SCHOOL DISTRICT INCOME TAX RETURN(S) (SD100)

To determine if the address(es) where you lived during the calendar year(s) have a school district income tax in effect, you may use the Finder at tax. and click on the Finder link from the home page. Select School District Income tax and lookup tax rate by address. Enter the address(es) where you lived during the calendar year(s) to find the school district name, number and tax rate. If the rate is greater than 0% you are required to file the SD-100 return(s).

PAYMENTS

The Tax Department is not authorized to set up a payment plan; however, any partial payments submitted will be applied to the outstanding balance. Partial payments will not stop the imposition of additional interest, penalty or the referral of the account to the Ohio Attorney General’s Office for collection. Please include a copy of the payment voucher with each payment mailed to credit the correct account and tax year or pay online at tax..

INTEREST

Ohio Revised Code 5747.08(G) requires interest to be paid at the rate set annually under Revised Code 5703.47, which is the federal short-term interest rate plus three percent. The interest rates can be located on our website at tax..

HOW IS INTEREST COMPUTED?

(Tax Due x Interest Rate) / 365 x the number of days late

Note: The denominator would be 366 if the tax was due in a leap year.

• Late filed or failure to file penalty – Ohio Revised Code, Section 5747.15(A)(1) the greater of:

▪$50 per month not to exceed $500 or

▪5% of the tax due per month not to exceed 50%

• Late payment or failure to pay penalty – Ohio Revised Code, Section 5747.15(A)(2)

▪Double the interest charged per Ohio Revised Code, Section 5747.08(G), which is explained above
