Long Term Disability Benefit Highlights

What is Long Term Long Term Disability pays you a portion of your earnings if you cannot work because Disability coverage? of a disabling illness or injury.

This highlight sheet is only intended to be an overview of your Long Term Disability coverage. An updated Summary Plan Description will be available after Jan. 1, 2017 on NetBenefits (thehartford) to explain your coverage in detail.

What is disability?

Disability is defined in The Hartford's Summary Plan Description. Typically, disability means that you cannot perform one or more of the essential duties of your occupation due to injury, sickness, pregnancy or other medical conditions covered by the insurance. Once you have received benefits for 24 months, you must be prevented from performing one or more of the essential duties of any occupation.

When am I eligible?

You are eligible if you are an active full time employee and you work at least 75% of the number of hours in The Hartford's normal work week but no fewer than 30 hours per week on a regularly scheduled basis. You will be eligible for coverage on the date you become an eligible person.

You must be on the United States payroll of The Hartford to be eligible. Temporary employees are not eligible.

How much coverage would I have?

The Hartford automatically provides coverage that would pay you a benefit of 50% of your monthly income loss to a maximum monthly benefit of $15,000 per month.

You may elect additional coverage as follows:

Option 1: You may elect to purchase coverage that pays you a benefit of 60% of your monthly income loss to a maximum monthly benefit of $18,000 per month.

Option 2: You may elect to purchase coverage that pays you a benefit of 70% of your monthly income loss to a maximum monthly benefit of $21,000 per month.

This plan includes a minimum benefit of the greater of: 10% of the benefit based on monthly income loss before the deduction of other income benefits or $100 per month.

When can I enroll? You can enroll for 60% or 70% coverage via NetBenefits (thehartford) or by calling The Harford HR Service Center at

Income Protect1i.o87n7.HR.AT.WORK. If you do not enroll for 60% or 70% coverage within 31 days after the date you first become eligible to do so:

(1) you will automatically be assigned basic coverage (50%); and

(2) you may not enroll for enhanced coverage (60% or 70%) until an annual enrollment period or until you have a change in family status. For details, refer to your Summary Plan Description.

When does my Coverage will be effective the date you become eligible. You must be actively at coverage start? work on the day your coverage takes effect.

How long do I have to wait before I can

receive my benefit?

You must be disabled for at least 26 weeks before you can receive a Long Term Disability benefit payment.

Are there other limitations to enrollment?

Evidence of Good Health is required if you have basic coverage (50%) and elect to increase your coverage to the 70% option.

How long will my disability payments

continue?

If you become disabled prior to age 63, as long as you remain disabled, benefits may continue until you reach your Social Security normal retirement age, or 42 months if greater. If you are disabled at age 63 or after, the duration of benefits is determined by a schedule. Disabilities due to mental illness and substance abuse are limited to 24 months. Retirement benefits that are funded by your after-tax contributions Your personal savings, investments, IRAs or Keoghs Profit-sharing Most personal disability policies Social Security increases

Exclusions: You cannot receive Long Term Disability benefit payments for disabilities that are caused or contributed to by:

War or act of war (declared or not) The commission of, or attempt to commit a felony An intentionally self-inflicted injury Any case where your being engaged in an illegal occupation was a contributing cause to your

disability

Mental Illness, Alcoholism and Substance Abuse: You can receive benefit payments for Long-Term Disabilities resulting from mental illness, alcoholism and substance abuse for a total of 24 months for all disability periods during your lifetime. Any period of time that you are confined in a hospital or other facility licensed to provide medical care for mental illness, alcoholism and substance abuse does not count toward the 24 month lifetime limit.
