Scholarship

Following is an application for the John H. Lyons, Sr. Scholarship. Please be sure to read the

instructions carefully and note that June 1, 2016, is the deadline for receipt of all materials

related to your application. It is your responsibility to be sure the application, along with all

supporting documents, including the Secondary School Report, are completed and returned by

the June 1st deadline. Applications received after the deadline may not receive consideration by

the committee of judges. However, the deadline may be extended at the discretion of the trustees.

As a part of the application procedure, students in the United States must provide college

aptitude test scores from one of the two major aptitude testing programs, either the SAT of the

College Entrance Examination Board or the ACT of the American College Testing Program. If

you have taken one or more of these tests, the Committee will give you the benefit of your higher

score. If you participated in the SAT, you may have your scores forwarded by logging on to

and following the prompts given. You will need to enter the school

code of 0554. If you participated in the ACT, you may have your scores forwarded by logging

on to and following the prompts on the link at the top of the page. You

will need to click on scores and scroll down to have your scores sent. The school code for the

ACT is 0661. If these scores are part of your high school record and will be forwarded with your

transcript by the office of your principal, academic adviser or guidance counselor, this will

satisfy this requirement. However, if these test scores are not included on your transcript, it is

your responsibility to provide a clear, legible copy or have them sent by the appropriate testing

service. No application of students in the United States will be considered without SAT or ACT

scores.

We are aware that Canadian applicants do not have nationwide access to SAT or ACT testing.

Therefore, Canadian students, only, are exempt from this requirement and judging guidelines

will be appropriately scored without penalty in this area.
