- most = Rheumatic heart disease

- thickened, calcified leaflets

- anterior mitral leaflet has characteristic ‘hockey stick’ appearance (long axis view) -> fusion and reduced mobility @ tip but intact cusp mobility

- tight ‘fish mouth’ appearance in short-axis view

Severity Assessment

- LA dilation

- AF

- area: < 1cm2 (normally 4-6cm2) determined from pressure half time (valve area = 220/pressure half time)

- gradient: > 10mmHg

- pulmonary hypertension
