All M4A1s are to be re-inspected with the additional function check attached (previously published in SOUM 18-005).Units will report M4A1 failures to TACOM utilizing mailbox provided below and shall update Modification Management Information Systems(MMIS) for 100% compliance. TACOM will supply a replacement selector for all weapons that fail the additional function check as well as selectors held as shop stock.This message and re-inspection is limited to M4A1's only.The weapon will perform as intended when utilized in IAW the Operator?s Manual utilizing current selector (unmodified).Units shall not modify components of weapon in order to remedy issue or for any reason outside of an approved Modified Work Order (MWO).Units shall follow selector removal and replacement procedures outlined in TM 1005-319-23&P (WP21). This routine task (remove and replace selector) is specifically authorized to a 91F only (Small Arms Repairmen). The existing procedures are correct.Upon request and by exception, a team will be sent to assist units in removal and replacement of selectors. A request for assistance should be sent to the Individual Weapons Team mailbox (usarmy.detroit.tacom.list.ilsc-individual-weapons@mail.mil), if needed.A team will NOT be sent for assistance with inspection and additional function check.1. Receipt Acknowledgement:?This is an Informational Ground Precautionary Action manders/Directors of Army Commands (ACOM)/Army Service Component Commands (ASCC)/Direct Reporting Units (DRU), Army National Guard (ARNG), US Army Reserve (USAR) Command, US Navy (USN), US Air Force (USAF), US Marine Corps (USMC) and other Service Commanders and Responsible Offices will retransmit this message to all subordinate Commanders/Activities within 24 hours of receipt and will, within 3 working days, acknowledge receipt for all ground safety and maintenance messages using the Modification Management Information System (MMIS)2. Compliance Reporting:?Commanders/Directors of Army Commands (ACOM)/Army Service Component Commands (ASCC)/Direct Reporting Units (DRU), Army National Guard, US Army reserve Commands and other Responsible Offices, will track unit compliance to the tasks outlined in this message in MMIS until 100% complete. Units shall report compliance not later than 30 Days from the release date of the message. Summary of Problem: On 29 MAR 2018 SOUM #18-004 alerted the field of an unintended discharge on an M4A1 that occurred when the operator pulled the trigger with the selector switch between the SEMI and AUTO detents (outside of detent). The weapon did not fire when the operator pulled the trigger and instead fired when the selector was moved further. On 23 APR 2018, SOUM #18-005 alerted the field, as a result of this incident, on-going investigation determined that there is the potential for all carbines and rifles to behave in this way. Both messages required performing a modified functions inspection and reporting those findings.b. Expected results if failure occurs: Unintended Discharge.5. User Actions:?a. Re-inspection of all M4A1s are required. Units that report failure of the modified function check will be sent a replacement selector lever within 6 months of GPA release. The new selector lever will be marked with an "F" on the face of the left side of the selector.Units shall still continue to perform additional function check outlined in embedded document (and contained in SOUM 18-005) prior to live fire. In addition, mandatory replacement of selector is necessary if personnel replace any of the internal fire control components in the lower receiver (Unmodified selectors that pass the additional functions check could fail in the future if internal firing components are replaced by routine maintenance).b. TACOM Approved Sustainment Maintenance FacilitiesDepot Level (modification of selector)Field (removal and replacement of selector switch)c. Failure reporting due 30 days from message release. TACOM will provide replacement selectors up to 365 days after release of GPA.After that timeframe, new selectors can be ordered through thesupply system. USAR and ARNG may request an extension to thistimeframe.d. Units shall not modify components of weapon in order to remedy issue or for any reason outside of an approved Modified Work Order (MWO).Unmodified selectors, NSN 1005-01-585-6042, can still be used until swap; operators need to ensure that the selector is fully seated in a firing position prior to attempting to fire. All new production M4A1 deliveries will be retrofitted with modified selector and pass function checks. TACOM team will coordinate with units to provide modified selectors as well as disposition instructions for M4A1 failures.Re-inspection of all M4A1s and reporting requirements are required. Units shall still continue to perform additional function check prior to live fire and any time new internal components of receiver are installed, until new selector is installed. To clarify, new selectors are either modified selectors marked with a "F" or new selector identified by NSN 1005-01-673-4242.Replacement and reporting is limited to M4A1s only.e. All M4A1 carbines are considered Fully Mission Capable (FMC) regardless of the results of the additional function check. Current selectors are deemed safe when operated IAW -10 technical manual.Modifying the selector has shown to eliminate unintended discharge fault. A tolerance stack has been shown to be root cause of issue; this has been eliminated by a modification to the M4A1 selector. Unit shall NOT attempt to modify themselves.NoteInstructions to access video of the additional function check and removal and replacement of selector can be found in attachment, "Access Video".Attachment List:. Unit Commanders:?a.- Ensure that all carbines are operated in accordance with TM 9-1005-319-10.- Ensure the weapon is always pointed in a safe direction.- Operators shall ensure that the selector is fully seated in a firing selection position prior to attempting to fire.- Ensure all unmodified selector (1005-01-585-6042) shop stock/bench stock is turned back into supply system. TACOM will offer a one for one swap. Unit will receive the new modified selector(1005-01-673-4242) in exchange. Unit should contact the Individual Weapons mailbox listed below for disposition instructions.- Ensure all M4A1s are re-inspected utilizing additional functions check within 30 days of message release.- Ensure unit does not attempt to modify selector.- Ensure units continue to perform additional function check prior to live fire.- Ensure the unmodified selector is replaced with a new modified selector if ANY internal component of the lower receiver is replaced during routine maintenance.- Ensure all failed selectors are sent back to TACOM for modification (one for one swap) upon receipt of newly modified selector; disposition will be provided by TACOM.- Report all failures to TACOM team mailbox:usarmy.detroit.tacom.list.ilsc-individual-weapons@mail.rmation should include: Unit name, location, UIC, weapon serialnumber(s) that failed, and total weapons inspected.b.
