I DO NOT hold a valid TN Teacher License and will make application as indicated below. Each type of license notes what I am required to provide to CMS at the time I process my new hire paperwork.

Apprentice License ? I have completed a teacher preparation program at a TN college/university. The program was completed at ___________________________________ I must provide a copy of the actual license application made to the TDOE or a letter from my college/university acknowledging that the application has been made.

Out of State License ? I have completed a teacher preparation program at a TN college/university outside of TN and I meet the requirements to hold a TN teaching license. I must provide a copy of the actual license application made to the TDOE or have the required documents needed to make application. The application is on the TDOE website. A link is provided for the guidelines and applications on this page.

Interim B License ? I have completed a teacher preparation program, but I have not completed all of the required PRAXIS test(s) for TN. I must provide a letter from a college/university acknowledging my program completion and recommending me for an interim B license. I understand that I am required to take the needed test(s) within the school year and this license is issues for a period of one year. I must provide an official transcript from all colleges/universities I have attended to be sent to the TDOE. For information and/or to register for the test(s), go to PRAXIS.

The following licenses are issues for a one year period and renewal requirements, as mandated by the TDOE, must be met for subsequent years:

Transitional License ? I have met the content knowledge requirements. I must provide official transcripts from all colleges/universities I have attended to be sent to the TDOE. I am required to contact a TN college/university regarding admittance to an approved transitional licensure program.

Transitional License Renewal ? I have been admitted to a teacher preparation program, successfully completed the orientation component of the core, completed three (3) semester hours of coursework form my program of study AND taken all required PRAXIS exams, except the PLT, for the endorsement(s) sought. I must provide an official transcript, program of study from my college/university that reflects the completed three (3) hours and the orientation component, and copies of scores form the required PRAXIS exams.

Other ? None of the above options describe my license status. A representative from Human Resources will discuss my license situation with me at the time I am in the office to process my new hire paperwork. I must provide an official transcript from all colleges/universities I have attended to be sent to the TDOE.

Interim B and Transitional Licenses are applied for by Collierville Schools. You will be asked to sign an application for licensure at the time you process your new hire paperwork. If CMS is applying for one of these licenses, an additional set of official transcripts must be submitted. One set will remain in your personnel file and one set will be submitted to the TDOE.

I am aware that I am required to hold a valid TN Teaching Certificate and the tit is my responsibility to comply with any request for information that I receive from the TDOE regarding the issuance of my license. I understand that if I am issued a Transitional License or an Interim B License that it is valid for one year and that I will be non-renewed at the end of the school year.
