In appreciation of your patronage as a current lessee of Nissan Motor Acceptance Corporation (NMAC), we are pleased to offer this Nissan Owner Loyalty waiver. Eligibility is determined by purchasing or leasing a new, unused, and unregistered Nissan, financed through NMAC within 120 days prior to your lease termination date or up to 30 days after lease termination.

PROGRAM SUMMARY ? Upon receipt of the completed, signed form, and verification of eligibility, we will waive up to $500 of Excess Wear and Use charges from the liability balance, if applicable. ? The name(s) on the new NMAC account must be the same name(s) as the lessee(s) of the previous Nissan vehicle. ? Only one Owner Loyalty Waiver offer per each Nissan account. ? The Owner Loyalty Waiver offer may not be used in lieu of any monies owed to or billed by NMAC on an existing or future lease or purchase contract, and may only be used toward the waiver of Excess Wear and Use. ? The Owner Loyalty Waiver offer is non-transferable.

INSTRUCTIONS:

1. Return this form in the enclosed envelope along with your remittance coupon and payment of the lease liability amount, if applicable.

2. Once this offer is processed, you will receive an updated liability statement. 3. Please make a copy of this form and retain for your records.

For leases owned by Nissan-Infiniti LT, Nissan Motor Acceptance Corporation acts as servicer.
