FDA APPROVED PRESCRIPTION TOPICALS Ciclopirox 8% (Penlac) is a medicated nail polish. It is painted on the infected nails and surrounding skin once a day. Every seven days, the piled-on layers are wiped off with alcohol. Efinaconazole 10% (Jublia) comes in liquid form. Daily application for 48 weeks resulted in cure rates of up to 18%. ORAL ANTIFUNGAL DRUGS Terbinafine (Lamisil), itraconazole (Sporanox), Fluconazole (Diflucan), and griseofulvin do not always work and carry side effects ranging from potentially life-threatening skin rashes such as Stevens Johnson syndrome to liver damage and jaundice. Clinically apparent liver injury due to terbinafine occurs in 1 case per 50,000 to 120,000 treatments. Terbinafine tends to be the most effective option in many cases. The daily dose is 250 mg. Treatment duration is 6 weeks for fingernails and 12 weeks for toenails. A pulse regimen at a dose of 250 mg for 1 week per month every 3 months is not FDA approved.
