Work Week: Sunday – Saturday

 

Holiday and overtime rates: SINS 621-001 through 621-026 receive straight pay only – no overtime. Holiday and overtime rate applies to: SIN 621-030 (PA), 621-053 (CRNA), and 621-054 (NP). For these SINS, add 1.1x the regular hourly rate for OT and 1.1x the regular hourly rate for Federal Holidays.

On-Call is defined as a physician being required to carry a pager to be available for consultation relative to specific specialty. While on-call the physician must remain within 30-mile radius of the facility that is being covered. On-call rates do not apply to a physician being required to be at or stay at a facility after hours.

Call back is when the physician is required to physically be at the facility and/or see patients. Normal hourly rates shall apply.

Weeknight On-Call is defined as a 15.5-hour period (ie, Monday – Friday 4:30p – 8:00a). The weeknight on-call rate (An additional $50 above the hourly rate) will apply.

Weekend On-Call Rates is defined as a 24-hour period either Saturday 8am – Sunday 8am or Sunday 8am – Monday 8am. The weekend On-Call rate (3 x hourly rate) will apply if a Physician is assigned to carry a pager to be available for consultation relative to specific specialty, to report to the facility and/or see patients on the weekend as described in the Statement of Work (i.e. Saturday 8am – Monday 8am).

Minimum Qualifications of Professionals:

 

SIN 621-001 through 621-024 Physician Minimum Qualifications:

Each Physician will possess: A Medical (MD) or Osteopathic (DO) degree. Completion of a Residency Training Program in their specialty approved by the Accreditation Council of Graduate Medical Education (ACGME). Completion of a Fellowship Training Program in their subspecialty approved by the Accreditation Council of Graduate Medical Education (ACGME), if required for certification. Board Certification or Eligibility by their specialty American or Osteopathic Board. A current, valid medical license in one of the U.S. states, territories, or commonwealths. Must have one year of experience in their respective specialty within the preceding 3 years.

 

SIN 621-003 Pulmonary Critical Care Physician Minimum Qualifications:

Physician will possess: Credential Awarded: American Board of Internal Medicine (ABIM) Certification in both Pulmonary Disease and Critical Care Medicine.

Eligibility Criteria: Hold a current, active medical license in a state or territory of the United States or hold the professional, legally recognized equivalent in another country. Hold a doctoral degree from a pulmonary and critical care program accredited by the Accreditation Council for Graduate Medical Education (ACGME) and certified by the American Board of Internal Medicine (ABIM). Must have one year of experience in their respective specialty within the preceding three year.

SIN 621-026 Dentist, General, Periodontist, Prosthodontist, Endodontist, or Oral Surgeon Minimum Qualifications:

DDS or DMD degree from an ADA accredited program. Minimum one year experience in specialty and must possess certificate of training in specialty from ADA. Current unrestricted state license. BLS required.

SIN 621-053 Certified Nurse Anesthetist Minimum Qualifications:

A Bachelor of Science in Nursing degree and a Masters Degree in Anesthetist. Certification in their specialty by the American Association of Nurse Anesthetists. A current, valid license in one of the U.S. states, territories, or commonwealths. Must have one year of experience in their respective specialty within the preceding 3 years.

 

SIN 621-030 Physician Assistant Minimum Qualifications:

Must have 1+ years experience in the previous 24 months. Must have an Associates or Bachelors. Must be certified by the American Academy of Physician Assistants. BCLS

 

SIN 621-054 Nurse Practitioner Minimum Qualifications:

Education: Undergraduate degree, Graduate Degree

Experience: Minimum of twelve months experience in respective specialty, preferred.

Certification: BLS Certification

Licensure: Unrestricted state licensure

Subspecialty: Additional training as part of the 2 year Graduate Degree with emphasis on that specific subspecialty.

Other: Additional experience and qualifications as outlined by the ordering service.
