|Details of Service |1. What Service Does the Unit Provide? |

| |The service provides specialist assessment and management of adult patients between the ages |

| |of 18 to 65 yrs, who suffer from mental illness. This includes interdisciplinary assessment |

| |of biological, psychological and social needs leading to the formulation of an |

| |interdisciplinary care plan designed for the individual patient. The service liaises with |

| |third party providers of services to optimise the opportunities available to individual |

| |service patients to avail of resources within their own communities and in so far as is |

| |possible to maintain their independence. The service works closely with other mental health |

| |teams, General Hospital Services and General Practitioners to ensure an integrated seamless |

| |service. |

| |2. What Client Group is served by the Unit? |

| |The service is a specialist mental health adult service to provide a service to those aged 18|

| |to 65 yrs. The service is provided to those who suffer from mental ill health and who |

| |require a specialist mental health service. |

| |3. What are the Future Developments for the Service? |

| |To develop the service in line with the requirements of “A Vision for Change” and ‘Primary |

| |Care a New Direction’. |

| |4. What is the Team Structure? |

| |The team comprises Consultant in General Adult Psychiatry, SHO x 2, C.M.H.N.x 4, 0.5 WTE |

| |CNM3, 1 Clinical Psychologist, 1 WTE Social Worker, 1 WTE Medical Secretary, 0.5 WTE |

| |Occupational Therapist, 1 WTE Family Therapist, and 1 HCA. |

| |Carlow North sector, Carlow/Kilkenny Mental Health Services. |

| |The organisational role of the consultant relates upwards through the Executive Clinical |

| |Director. |

| |Working Relationship: |

| | |

| |The working relationship requires co-ordination, liaison and collaboration with other health |

| |care professionals, managerial/administrative staff and agencies associated with standards |

| |and inputs to the provision of care for clients of the service. |

|Purpose of the Post |The consultant’s position is both clinical and organisational (in the service context). |

| | |

| |The clinical role is diagnostic, investigative, treatment and prescription, either directly |

| |or through others but involving continuing full responsibility for the treatment of patients |

| |without supervision in clinical matters. |

| | |

| |The organisational role carries service planning and resource management responsibilities at |

| |individual consultant level and at the department/unit and hospital levels. |

| | |

| |The teaching role arises from the clinical and organisational dimensions of the role and |

| |focuses on health staff training and development. Consultants will contribute to the |

| |training, education and supervision of non-consultant hospital doctors, undergraduate |

| |students and a range of clinical staff. |

| | |

| |The effectiveness of the role will be seen in the quality of care provided within the |

| |parameters of policy and the optimum use of available resources. |

| | |

| |The consultant shall engage in health services accreditation process. |

|Principal Duties and |The provision of consultant services will be in accordance with the agreed schedule of |

| |Duties: |

| | |

| |In exercise of his/her powers under subsection (3) of Section 14 of the Health Act, 1970 the |

| |Chief Officer will determine that the person appointed shall perform the following duties |

| |appropriate to the office; |

| | |

| |Consultant Adult Psychiatrist within Carlow/Kilkenny Mental Health Services, Health Service |

| |Executive South and in particular: |

| | |

| |To be responsible for the care and treatment of patients in his/her clinical care. |

| | |

| |To attend, persons receiving services in any health institution maintained by the HSE and to |

| |advise on their treatment as required by the Chief Officer/Clinical Director |

| | |

| |To attend as directed by the Chief Officer/Clinical Director, at clinics and out-patient |

| |departments maintained by the HSE and to provide either thereat or in the appropriate |

| |hospital a diagnostic treatment and consultation service as may be appropriate for or in |

| |respect of patients for whom the HSE is obliged under any enactment to provide such a |

| |service. |

| | |

| |To be responsible, under the person in charge, for the general management and running of such|

| |departments of the hospitals of the HSE, or of such other psychiatric services operated by |

| |the HSE, as may be assigned to him/her and to be responsible for the care and treatment of |

| |the patients concerned in such assignment. |

| | |

| |To deputise, if required, for the person in charge when he/she is absent or the office is |

| |vacant |

| | |

| |To arrange for the discharge from hospital of all patients in his/her clinical charge for |

| |whom hospital care and treatment, in his/her opinion, is no longer necessary. |

| | |

| |To take special interest in and to be responsible, under the person in charge, for the |

| |general organisation of mental health services for the patients of the HSE as may be assigned|

| |to him/her. |

| | |

| |To co-operate and liase with the HSE consultants and provide consultant advice and guidance, |

| |on request, or as appropriate. |

| | |

| |To keep such records as may be required from time to time by the HSE. |

| | |

| |To co-operate with the Chief Officer, Local Health Manager, Medical, Nursing and Paramedical |

| |staff in all matters appertaining to the general efficiency and effectiveness of the HSE |

| |services, and attend case conferences as requested. |

| | |

| |To furnish on request, to a registered Medical Practitioner authorised by the HSE or the |

| |Minister, the clinical details regarding any person who is or has been under his/her care in |

| |the hospital or clinic on production of the written consent of the person (or written consent|

| |of the person’s representative or next-of-kin). |

| | |

| |Whenever the Minister is of the opinion, however, that it would not be in the interests of |

| |the common good to seek such consent and he/she certifies accordingly, the officer shall |

| |furnish the required information to the registered Medical Practitioner authorised by the |

| |Minister. Where, however, the officer is of the opinion that the nature of the case is such |

| |that he/she should inform the patient of the requirement of the Minister, he/she shall be at |

| |liberty to do so. |

| | |

| |It is not intended that this requirement will operate in a manner which would detract from |

| |the patient’s character or reputation. |

| | |

| |To examine any person referred to him/her by the HSE for examination and report and to |

| |furnish to the HSE a full report of his/her findings on such examination, unless the person |

| |referred informs the Officer that he/she objects to the examination or to the submission of |

| |such reports, in which event the Officer shall notify the HSE. |

| | |

| |To provide postgraduate education for Non-Consultant Hospital Doctors, General Practitioners,|

| |Area Medical Officer, Nursing Staff and Paramedical Staff. |

| | |

| |To supervise and be responsible for the clinical work and record keeping of Non-Consultant |

| |Hospital Doctors and other health professionals under his/her control and authority and to be|

| |responsible for the proper performance of their duties by the staff assigned to the |

| |department/speciality and services under his/her control and to instruct them in the proper |

| |performances of their duties both by means of practical instruction and lectures. |

| | |

| |To notify the appropriate officer of any case of infectious or suspected infectious disease |

| |in his/her department of the hospital or area of responsibility and to furnish him/her with |

| |such particulars as he/she may require in regard to such case and in conjunction with the |

| |said officer to carry out preventive measures as may be necessary. |

| | |

| |To issue, or arrange for the issue of, to or in respect of any patient under his/her care in |

| |a hospital, or referred to him/her at a clinic, and without charge any certificates which |

| |might reasonably be required by such patient in regard to the state of his/her health in |

| |connection with his/her normal employment of his/her entitlement to benefit under any scheme |

| |of social insurance or assistance. |

| | |

| |To be responsible for the maintenance of appropriate liaison arrangements between his/her |

| |services and the patients normal medical attendants and nominated next of kin. |

| | |

| |To participate in structured arrangements for the determination of resource provision, |

| |utilisation, service planning, evaluation and performance review. |

| | |

| |To participate in representative and collaborative working arrangements put in place by the |

| |Health Service Executive South. |

| | |

| |To negotiate and agree with the employer a Schedule of Commitment and Practice Plan that is |

| |consistent with the optimum use of available resources and facilities and with the schedule |

| |of Commitments and Practice Plans of colleagues |

| | |

| |To set up and participate in formal audit |

| | |

| |To participate in Hospital Accreditation process. |

| | |

| |To participate in mandatory health and safety training |

| | |

| |To provide cross cover for short term absences when locums are not available |

| | |

| | |

| | |

|Eligibility Criteria |Each candidate must have the following on the latest date for receiving completed forms. |

|experience |No candidate will be appointed as a Medical Consultant unless (s)he is registered as a |

| |Specialist in the Specialist Division of the Register of Medical Practitioners maintained by |

| |the Medical Council of Ireland. |

| |Successful candidates must be registered as a Specialist in the relevant Specialty on the |

| |Specialist Division of the Register of Medical Practitioners maintained by the Medical |

| |Council of Ireland within 180 days of the day of interview. |

| |Should the successful candidate not be registered as a Specialist at that time, the PAS may |

| |choose not to recommend that candidate to the employer and the post may be offered to the |

| |next suitable candidate. Should no suitable candidate exist, a further recruitment process |

| |may be initiated. |

| |(2) Health |

| |A candidate for and any person holding the office must be fully competent and capable of |

| |undertaking the duties attached to the office and be in a state of health such as would |

| |indicate a reasonable prospect of ability to render regular and efficient service. |

| |(3) Character |

| |Each candidate for and any person holding the office must be of good character |

| |(4) Age |

| |Age restriction shall only apply to a candidate where he/she is not classified as a new |

| |entrant (within the meaning of the Public Service Superannuation Miscellaneous Provisions Act|

| |2004). A candidate who is not classified as a new entrant must be under 65 years of age on |

| |(5) Entry to competition/recruitment process |

| |For the purposes of eligibility for entry to any competition or recruitment process |

| |associated with this post, applicants currently in employment as Senior or Specialist |

| |Registrars in HSE or HSE-funded agencies may participate in the competition on the basis |

| |that, on the latest date for receipt of applications, they are within 180 days of |

| |from the relevant recognised postgraduate medical training body in writing. |

Terms and Conditions of Employment

|Tenure |The appointment is Locum on an as and when required basis. |

| |Appointment as an employee of the Health Service Executive is governed by the Health Act 2004|

| |and the Public Service Management (Recruitment and Appointment) Act 2004. |

| | |

|Remuneration |Revised salary scales for appointments after 01.10.12 will apply |

| |The standard working week applying to the post is 39 hours. |

|Working Week |This 37 hour commitment will normally be delivered across a span of 12 hours between the |

| |hours of 8a.m. and 8p.m. Monday to Friday. |

| | |

|Annual Leave |Pro Rata - The annual leave associated with the post is 31 days per annum and as determined |

| |by the Organisation of Working Time Act. |

| | |

| | |

|Superannuation |All pensionable staff become members of the pension scheme. |

| |at more than one location will be re-imbursed expenses in respect of travel between locations|

| |specified in the schedule which are agreed with the employing authority or authorities. |

|Special Terms and Conditions |As this post is one of those designated under the Protection of Persons Reporting Child Abuse|

| |Act 1998, appointment to this post appoints one as a designated officer in accordance with |

| |Section 2 of the Act. You will remain a designated officer for the duration of your |

| |appointment to your current post or for the duration of your appointment to such other post |

| |as is included in the categories specified in the Ministerial Direction. Such officer will |

| |on receiving a report on child abuse formally notify the Senior Social Worker or designated |

| |officer. |

| | |

| | |

| |Indemnity against Negligence etc. |

| | |

| |Personal injury claims against the holder of a consultant post in a public hospital arising |

| |from the treatment of public or private patients in the hospital are covered by the Clinical |

| |Indemnity Scheme. The scheme was established under the terms of the National Treasury |

| |Management Agency (Delegation of Functions) Order 2003. |
