At Rochester Regional Health, we want to support and encourage employees to grow in their current roles and develop the skills and experiences to qualify for new challenges and opportunities throughout their careers with us. One important way we do this is through our Tuition Assistance Program.

The Tuition Assistance Program reimburses certain employee education costs associated with career development at Rochester Regional.

Who's Eligible?

? Benefit-eligible employees who are regularly scheduled for 20 or more hours per week are immediately eligible.

? Employees must not be on a leave of absence or under disciplinary action. ? If an employee experiences a job status change while taking a course,

the employee's status at the time of course completion will be used to determine the amount of eligible assistance (please refer to the Maximum Annual Assistance Levels chart below). ? If an employee's employment has been terminated prior to expected tuition assistance payment, no assistance will be paid.

What Courses Qualify for Tuition Assistance?

? Undergraduate or graduate courses related to an employee's current position or a future position within Rochester Regional Health.

? Final grade must be a "C" or above or "Pass" if only a pass/fail class. ? Courses must be completed at accredited schools, colleges

and universities.

What Expenses are Eligible for Reimbursement?

? College-accepted challenge exam fees ? Tuition ? Laboratory fees

NOTE: Prior to reimbursement, a Tuition Assistance Application must be completed and accepted. Applications are available in Workday under "Benefits and Retirement."

NOTE: Any partner tuition discounts received are in addition to approved tuition assistance provided by Rochester Regional Health.

Prorated amounts based upon standard hours worked. (For example, employee working 20 hrs/wk is eligible for $1,500 max/yr for undergraduate courses).

NOTE: Other financial aid, discounts, scholarships and grants (not including loans) are deducted from the eligible assistance amount. For example, a student with $5,000 tuition who receives a state or federal grant/scholarship of $2,000 may only apply for $3,000 of assistance. Maximum assistance may not exceed your out-of-pocket tuition costs.

Rochester Regional Health adheres to Section 127 of the Internal Revenue Code.

How to Participate

Following are the steps required to participate in the program:

1. Complete and submit the Tuition Assistance Program application no earlier than 30 days prior to and no later than 15 days after the start date of the semester or the course (application available in Workday). Please be sure the application includes the following: ? Acceptance letter or school transcript1 from the school, college or university. ? Official course registration schedule from the school with the institution's name, student's name, list of courses and course numbers with start and end dates and number of credit hours awarded for each.

2. The Benefits Department will review the application and notify you of participation acceptance or denial within 10 business days.

3. Within 30 days of course completion, you must submit proof of a passing grade and an itemized billing statement to the Benefits Department that contains the details outlined below. Incomplete information or requests received past the 30-day period will not be processed and therefore not eligible for reimbursement. ? Official documentation from the school that includes the institution's name, student's name, course name(s), number(s) and final grade (legible screen shots are accepted if they include all of the required details). ? Itemized bill from the school that includes the institution's name, student's name, tuition amount, school discount and any financial aid received (legible screen shots are accepted if they include all of the required details).

4. The Benefits Department will review the documents and notify you of assistance approval or denial within 10 business days.

5. Approved assistance payments will be included in your paycheck as soon as administratively possible.

NOTE: Assistance payments are applied to the year in which they are paid. For example, if you complete and pass a course in the fall semester, and submit the necessary reimbursement documents on December 31, the payment will not be made until January of the following year and will apply to the annual maximum of that year, even though the course was taken in the prior fall semester. All reimbursement requests must be submitted within 30 days of course completion.

Rochester Regional Health reserves the right to administer, interpret, revise, or revoke any or all of the provisions of this benefit at any time.
