|[pic] |The Contract Specialist (CS) Richard Worthy submitted a Request for Information (RFI) through WEBS on July 28, 2015 to get |
| |the most up to date market specifications. Four (4) vendors responded to the RFI. CS also requested DOC and WSP for |
| |persons interested in being part of the Stakeholder Group. |
| |July 30, 2015 three (3) organizations wished to be part of the stakeholder group. They were from the City of Everett |
| |Special Police, WSP, and DOC. |
| |August 22, 2015 CS received the latest specifications for the Mask and Clothing from three (3) vendors. |
| |September 15, 2015 CS sent the stakeholders the conflict of interest (CoI) form to sign and return. All stakeholders signed|
| |and returned the CoI. |

|Specification compliance? |Mallory Safety had not completed several columns on the Specification document. I contacted the point of contact and |
| |requested that they verify the Specifications. Mallory informed the CS that they have over looked some of the columns and |
| |would send me a corrected Specification document. Received the corrected document within 30 minutes of email. All others |
| |were completed and responsive. |
|Price Sheet compliance? |All four (4) bidders submitted responses in category “A” and only two (2) in category “B”. Pricing was not consistent. |
| |Both All Safe and Mallory bid box price instead of each. I contacted vendors and clarified. I updated the price sheet on |
| |line item 4 and 9 in category “A”. Accordingly – prices were not change, only divided by 6 (from box to each). The price |
| |adjustments did not change the winning bidder. |
|Other Responsiveness checks? |Two of three references reported back that they were very satisfied with the service of All Safe. The third reference did |
| |not respond. Two of three reference replied to the questionnaire with a very good response of Mallory Safety. The third |
| |reference did not respond. |
|Bid Evaluation—Responsibility |
|Past Performance? |All Safe is a current vendor on contract 06715, Technical Rescue Equipment. All Safe has delivered products, reported |
| |sales, and paid fees on time. Mallory Safety & Supply had contract 07608 Structural Bunker Gear and acquired Sanderson |
| |which has contract 07914 Rainwear. Mallory has delivered product on time, reported and paid management fees. No |
| |performance issues have been noted. |

|Evaluation:[pic] |The lowest total price per category received an award. Mallory Safety is lowest responsive responsible bidder in Category |
| |“A” (Masks) and All Safe is the responsive responsible bidder in Category “B” (Clothing). |
|Results & Recommendation |
|Savings: |Through competition there as a 6.5% difference between the lowest and highest bidder. The contract saves .91% on the 3M |
| |Breathe Easy unit. No other savings was noted. |
|Recommendation: |Based on the facts above, it is in the best interest of the state to award a Mallory Safety Category “A” (Masks) and All |
| |Safe Category “B” (Clothing). The procurement Coordinator will make an award to Mallory Safety and All Safe. |
