SELECTIVE SERVICE REGISTRATION VERIFICATION

In order to receive Federal Student Aid, you must be registered with the Selective Service if you are a male who is at least 18 years old and born after December 31, 1959. Because the processor of your FAFSA application did not confirm that you registered with

Selective Service, you must now clear this up with the Financial Aid Office before your student aid may be processed.

Check the one that applies to you and provide proof of Selective Service Registration:

I am exempt because I'm a Female. [no copies are needed]

I am exempt because I was born before December 31, 1959.

I am under the age of 18.

I am registered with Selective Service.

? Attach proof of your registration status. You can obtain a copy from or call 847-688-6888.

I am not registered, but I am between the age of 18 and 25.

? Please visit the Selective Service Website: WWW. to register with Selective Service and attach a copy of the confirmation page to this form, OR

? You can request to be registered through your FAFSA application by logging into and submitting a correction

I am not registered, but I am on a valid non-immigrant visa.

? For example, if you entered the U.S. as an F-1 student visa and remained in that status until your 26th birthday, you must provide documentation indicating you were admitted on an F-1 visa and attended a school full time as required, i.e. I-20 form or a letter from the school attended indicating full time attendance as a non-immigrant alien.

I am not registered, and 26 years of age or older; but I am a Veteran or currently active duty

? Attach a copy of the DD-214 or current full time active duty orders showing military service in the armed forces - other than the reserve forces, the Delayed Entry pool, and the National Guard. This is compelling evidence that failure to register was not knowing and willful.

I am not registered, but I was 26 years of age or older when I entered the United States.

? Provide copies of your documentation, i.e. Date of entry stamp in your passport, I-94 with date of entry stamp on it, or a letter from the U.S. Citizenship and Immigration Services (USCIS) indicating the date you entered the United States.

? If you entered the U.S. illegally after your 26th birthday, you must provide proof that you were not living in the U.S. from age 18 through 25. Resident Alien Card (Green Card) is NOT valid as proof of the date of entry to the United States.

I am not registered, but I have received a Status Letter from Selective Service.

? Attach a copy of your Status Information Letter. If your Status Information Letter states that you were required to register but failed to do so, please attach an explanation of why you did not register (include where you lived between the age of 18 and 25 and why you were not aware of the widely publicized requirement to register when you were between 18 and 25 years old). You must demonstrate that you did not willingly or knowingly fail to register.

? If you do not have a Status Information Letter, you can obtain this letter by submitting a Request for a Status Information Letter to the Selective Service Office. Please go to to obtain and submit the Request for a Status Information to the Selective Service Office.

I certify that all of the inform ation provided by m e on this form is true and com plete, to the best of m y know ledge.
