Services: Long Term Care Unit Staff assess the needs of an older individual and develop a personal care plan of appropriate services and resources.

1. Family Caregiver Support Program- reduces the caregiver stress and reinforces the care being provided to older persons at home. Components of the program include a thorough assessment of the caregiving situation, the development of an appropriate care plan, benefits counseling, caregiver education and training, and financial benefits. Financial benefits are available for the reimbursement to caregivers for the purchase of ongoing caregiving supplies (e.g., adult undergarments) and services (e.g., respite) and for home modifications (e.g., ramps) and assistive devices (e.g., grab bars). Reimbursement amounts are dependent upon need, income and expenditures. The caregiver must reside with the related care receiver. Call for specific eligibility information.

2. Pennsylvania Department of Aging Waiver Program: is for people 60 and older, who have been medically assessed and determined eligible for nursing facility care but choose to remain in the community. Services are funded through a special waiver of certain Medicaid restrictions, allowing payments typically used for nursing home care to be used for home-care services. Eligibility is determined, then a caseworker designs a care plan with the individual and monitors appropriate services. Possible services that may be available: homemaker services, personal care, adult day care, home delivered meals, home environment modifications, medical supplies and equipment, registered nurse, transportation, caregiver respite services, and emergency response system. You may choose which local organization or company that you may want to provide the services. Call for specific eligibility requirements and limits to the amount of services available.

3. Adult Day Care- offers a supervised setting for frail and/or impaired older adults, age 60+, who are unable to be alone during the day. Open Monday through Friday, 8:30 a.m. to 4:30 p.m. Individuals participate in socialization, arts and crafts, life skills education, and preventative health care. Caregivers receive respite (relief) while their loved-ones attend the center.

4. Caregiver Support- caregivers of disabled adults can receive support from other caregivers through this support group. The group meets once a month in the ASI Community Center. Caregivers can learn coping and caregiving skills and gain knowledge about diseases, medications, research, and legal issues. The primary focus is caring for dementia victims.

5. Companion Liason- Aging Services, Inc. maintains and distributes a listing of individuals who want to work as companions to the elderly. The companions are not employees of ASI, but they have supplied references and completed an application

6. Home Delivered Meals- available to homebound consumers, Monday through Friday, who are unable to prepare meals and who do not have anyone available to prepare a meal for them. Meals are delivered in all areas of Indiana County except Indiana borough and White township.

7. Personal Assistance Services- personal care services for consumers 60+ who require help with daily living and hygiene needs.

8. Placement- assistance selecting an appropriate Personal Care Home for 24 hour custodial care and explanation of Personal Care Home Supplement.

9. Protective Service- assists consumers 60+ who are at risk of physical, psychological and sexual abuse, neglect, self-neglect, and/or exploitation.

10. Technical Assistance- aids in completing public assistance benefits forms.

11. Apprise- assists with health insurance complexities. Services include information on health insurance benefits, Medicare and Medicaid eligibility, claims filing, telephone assistance, consumer protection, referral to other resources, and advocacy.

12. Homemaker Program- furnishes light housekeeping, laundry, and shopping assistance to frail elderly living in their own homes.

13. Information and Referral- by phoning Aging Services, Inc., you will reach a contact person who can assist with any consumer issue or concern.

14. Legal Assistance- legal information is given through seminars and programs at the social centers and other facilities.

15. Ombudsman Program- an advocacy program to assist residents in long term care facilities within Indiana County.

16. Outreach- distributes information to the public on Aging Services, Inc., and other agencies

17. Senior Employment Program- acts as a broker, matching employers and those 55+.

18. Telephone Reassurance- frail consumers living alone can be phoned (Monday through Friday), to ensure their well being. Further steps are taken if the call is unanswered.

19. Transportation- provides rides to medical appointments in Indiana County for individuals 60- 64 who are not eligible for Medical Assistance and unable to access public transportation. Transportation to the social centers is provided to any individual age 60+ who cannot access public transportation because of a mental or physical disability. A 72 hour notice is required. Transportation is based on need and availability of drivers.

20. Social Centers – provide an opportunity for socialization, recreation, and educational programs. A hot lunch is available by contacting the Social Center 24 hours in advance to reserve the meal.

Fees: Cost sharing is applied to services received, based on income of consumer. There is a sliding fee scale.

1. Reach To Recovery “No One Should Have to Face Breast Cancer Alone”. Through face-to-face visits or by phone, Reach To Recovery volunteers provide support for:

a. people recently diagnosed with breast cancer

b. people facing a possible diagnosis of breast cancer

c. those interested in or who have undergone a lumpectomy or mastectomy

d. those considering breast reconstruction

e. those who have lymphedema

f. those who are undergoing or who have completed treatment such as

chemotherapy and radiation therapy

g. people facing breast cancer recurrence or metastasis (the spread of cancer

to another part of the body)

2. Look Good… Feel Better is a community-based, free, national service that teaches

female cancer patients beauty techniques to help restore their appearance and self image during chemotherapy and radiation treatments. Held at local hospitals, volunteer beauty professionals lead small groups, usually consisting of six to ten women, through practical, hands-on experience. Women learn about makeup techniques, skincare, nail care, and options related to hair loss such as wigs, turbans, and scarves. Each group program participant receives a free kit of cosmetics for use during and after the workshop.

3. Road To Recovery Getting to cancer treatments can be a great challenge especially for

our senior population. The American Cancer Society provides transportation for cancer patients to their treatments and home again. Most volunteer drivers can stay the ½ hour or 1 hour with the patient during their treatment, then take them home. Generally, call two weeks in advance to schedule a ride.

4. Man To Man is an American Cancer Society prostrate cancer education and support

program that offers community-based group education, discussion, and support for men with prostrate cancer.

5. Medication Assistance and Medical Supply Assistance These programs offer one-time

grants for cancer patients having trouble paying for their medication and supplies.

6. Wig Voucher $75.00 one time credit for a wig for cancer patients experiencing temporary hair loss.

Services:

The Armstrong-Indiana Mental Health/Mental Retardation Program was established by the County Commissioners of Armstrong and Indiana Counties to comply with the Mental Health and Mental Retardation Act of 1966. That act stipulates that the counties either separately or jointly provide a variety of community based services for the mentally disabled of their county. The Armstrong-Indiana MH/MR Program Administrator’s Office develops plans, allocates funds and has general oversight responsibilities for the two county program.

For older adults, the MH/MR Program assesses individuals who may require long-term treatment in a psychiatric setting. Our program does make referrals to other agencies if the individual does not require inpatient treatment and is able to handle a less restrictive setting.

The MH/MR Program facilitates the monthly meetings of the Community Support Program (CSP). These monthly meetings address the interests of the mentally disabled. CSP is represented by family members, consumers and all interested agencies. There are specific agenda items geared to older adults and their family caregivers.

MH/MR has oversight responsibilities in the area of Mental Health-Managed Care. This may include older adults and their family caregivers. The Managed Care organization and the Southwest Six Corporation represent the interests of Indiana County recipients of publicly-funded behavioral health.

MH/MR coordinates a quarterly meeting of the Suicide Task Force.

Fees: A financial liability is done through the Base Service Unit for outpatient services. However, there are no financial guidelines determined at the time there is a request for involuntary inpatient treatment.

1. Outpatient Services provides services to adults and the elderly. Services include individual therapy, marital and family psychotherapy, play therapy, group therapy and counseling for all ages.

2. Psychiatric Services include psychiatric evaluations and medication management by board certified psychiatrists and physician assistants.

3. Adult Partial Hospitalization is designed for adults who do not require full-time hospital care, but need continued support and therapy to maintain daily living routines. This program enables an individual to receive treatment for several hours a day up to five days a week.

4. Intensive Outpatient Program (IOP) is a short-term, acute program, less restrictive than partial hospitalization, but more intensive than individual outpatient therapy.

5. Psychiatric Rehabilitation is a program developed on the recovery model, providing vocational, educational, and other rehabilitative services.

6. Developmental Disabilities program services are designed to meet the needs of individuals with intellectual disabilities (mental retardation). Families are assisted in obtaining vocational/educational programs, home-based services, residential programs, respite care and advocacy.  The DD Unit also provides Early Intervention Service Coordination for children ages birth to 3.

7. Targeted Casemanagement is a program for adults and children that link the consumer to services such as housing within the community, medical care, education and employment when feasible. Provides 24/7 crisis care.

8. Resource Coordination is similar to Intensive Case Management but less intensive, and does not have a 24-hour component.

9. Family Based Services offers a comprehensive approach including Family Therapy, Family Support Services, Case Management, and 24 hour/ 7 day-a-week On Call services. Our goal is to empower multi-power families to gain skills and a sense of hope and control in their lives.

10. Wraparound/Strength Based programs are home and community based therapy services for children, adolescents and their families. Works with individuals and families with complex needs within their homes, schools or local community

11. Summer Treatment Program is for children ages 5-15 diagnosed with ADHD. This 8- week summer program works with children through the use of therapeutic recreational activities and a classroom component. The program is offered in 5 convenient locations.

Eligibility Requirements: Self referral and other referral accepted. Individuals must have a mental

health diagnosis for insurance coverage for mental health services.

Fees: Sliding fee schedule available for mental health services; most major insurances accepted.

Developmental Disabilities Unit services are provided at no charge to the family or individual.

Hours of Operation: Monday through Friday, 8:00 a.m. to 4:00 p.m.

Services: The Housing Authority provides affordable and/or Subsidized Housing for elderly,

handicapped or disabled individuals or low income families at 19 sites throughout Indiana

County. The Authority also administers the Section 8 rental assistance program for 520

households residing in privately owned rentals in Indiana County. Income restrictions apply at

some, but not all, properties. No appointment necessary.
