USING THIS DIRECTORY:

This Directory was designed for you by Parkview Health Plan Services (HPS). It contains descriptions of Signature Care network hospitals and ancillary providers, as well as alphabetical and specialty listings of Signature Care network healthcare providers. Please note that, while the information contained in this directory was accurate as of the date of publication, our agreements with providers are subject to change throughout the year. To make certain your physician, ancillary, hospital or other healthcare provider is currently a preferred provider, always ask the provider before you receive care or check online at . You may also check with the HPS Customer Service Department at (260) 266-5510, or (800) 666-4449 for physician, allied health professionals, ancillary or hospital participation.

The providers listed are recommended on the basis of their participation in Signature Care. Membership in Signature Care is not intended as a warranty or guarantee of any provider's skill or ability. Physicians in this directory are private practice physicians and are not employees of Signature Care or HPS. All participating physicians, allied health professionals and hospitals are independent contractors and not the agents of Signature Care.

POINTS OF INTEREST CONCERNING SIGNATURE CARE:

The Signature Care network is the result of efforts by Parkview Health and its affiliated healthcare providers to manage the cost of health care.

The cornerstone of Signature Care's network is Parkview Health and its affiliated healthcare providers in Fort Wayne and the surrounding area. In addition, other Indiana and Ohio healthcare providers, hospitals and ancillary providers have contracted with Signature Care as preferred providers. All contracted healthcare providers have agreed to offer their services at a reduced cost to Signature Care members. Please note that Signature Care is neither an insurance company, claims payor, nor a provider. It is a network of providers working in conjunction with your employer's health plan.

SAVING MONEY WITH SIGNATURE CARE:

When you need to see a healthcare provider, you can save money with Signature Care. By choosing a Signature Care healthcare provider, you pay less, not only through lower deductibles and co-payments, but also through the reduced fees Signature Care has negotiated with the provider. How much can you save? The amount and method of savings will vary according to your plan, but the dollars add up for every plan. For specific information, please refer to your company's Benefit Plan Description.

FREEDOM OF CHOICE WITH SIGNATURE CARE:

Freedom of choice is one of the biggest reasons Signature Care was created. Some health plans lock you into a mandatory system. With Signature Care you have the right to make your own choice.

Each time you decide to see a healthcare provider, you choose whether to use the Signature Care network or not. As stated earlier, if you choose a Signature Care network provider, there are savings involved for you and your employer. If you do not, your plan will process the claim according to established benefits, but you will not receive the same savings.

HOW TO USE SIGNATURE CARE:

When you visit a Signature Care healthcare provider, you must show your Signature Care identification card indicating your participation in Signature Care. By letting your healthcare provider know that Signature Care is part of your benefit plan, you can save money and your employer or group plan saves too.

For the greatest savings, use a Signature Care healthcare provider, ancillary or hospital every time. If an attending physician (or an emergency care physician) refers you to another doctor, you can still benefit by asking for a referral to a Signature Care participating physician or ancillary provider.

HOSPITAL AND OUTPATIENT PROCEDURE REVIEW:

The number for information regarding utilization management, including precertification requirements, medical necessity and benefits, will be identified on the back of your Signature Care identification card.

FILING CLAIMS:

Signature Care healthcare providers will automatically file claims for you. Depending on your employer's benefit plan, your physician may collect a co-payment from you at the time of your visit. After the claims are repriced by HPS, they are sent to your benefit plan payor. Your benefit plan payor pays the provider and the provider will bill you for balances due or deductible for any services not covered. If you have questions regarding claims payment, contact your benefit plan payor. Non-Signature Care providers may ask you to file your own claims and/or collect payment from you at the time of service.

For information concerning eligibility or specific plan coverages, contact your Employee Benefits Department or your benefit payor (insurance, TPA, trust). Signature Care PPO does not determine eligibility, benefits or pay claims, but acts in conjunction with your current health plan so you can access the lower costs within the network.
