JUROR QUALIFICATION QUESTIONNAIRE

, you have been identified as a prospective juror for the United States District Court for the District of Minnesota. Federal law requires you to complete a Juror Qualification Questionnaire to determine if you qualify to serve as a juror in Federal Court.

Complete the questionnaire online by going to mnd. and clicking on the "Jurors" tab on the welcome page and then clicking on the "eJuror" tab. You will need your 9 digit participant number which is located above your name on the front of this postcard. If you are unable to access eJuror for any reason, a paper copy will be AUTOMATICALLY mailed to you in approximately 8-10 weeks.

If you qualify, you may be randomly selected to appear for jury service. If so, you will receive a summons, along with additional instructions, informing you of the date and location of your jury service. PLEASE COMPLETE YOUR JUROR QUALIFICATION QUESTIONNAIRE ONLINE.
