|In one paragraph, describe genetic or biological factors related to the | |
|substance use. Include family history, medical sequelae, and psychiatric dx. | |

|Part III: Formulation | |
|Write a paragraph or two describing your conceptualization of the patient. | |
|With regard to your theory, conceptualize this client and his substance use | |
|disorder. Why did he develop his substance use disorder? What supported | |
|(maintained) the disorder? Give examples from the session. | |

|Part IV: Predicting Responses to the Therapeutic Situation (Write three | |
|paragraphs.) | |
|In the first paragraph, describe the issues or themes the counselor will want | |
|to watch for, related to the dynamic formulation. Also, describe what forms of | |
|resistance the counselor should watch for. | |

|In the second paragraph, describe what sorts of therapeutic interventions the | |
|counselor might plan to use in response to the issues/themes and the | |
|resistance. | |
