Supporting documents required
Please make one-sided copies (please do not use staples) of the following:
( Two years of W2s
( If self employed: a current profit and loss statement, two years of 1040s
( Your last paycheck stubs to cover one full month (if paid biweekly, we need 3 pay stubs.
If paid twice a month, 2 pay stubs).
( Two months bank statements, all pages, even if empty
□ Your recent statement of your 401k, stock brokerage accounts, IRA etc
□ A copy of your fire insurance policy that shows the name of the insurance company, name and phone
number of the agent, policy number, amount of coverage, and the replacement costs.
□ A recent mortgage statement for your ( 1st loan ( 2nd loan
□ HOA contact information (address and phone of association/management company)
( A copy of your driver license (150% enlarged, if possible)
If you keep your current 2nd loan, we need to have
□ A copy of your Home Equity Line of Credit mortgage agreement (multiple pages, says: DEED OF
TRUST on top)
( A copy of the note (multiple pages, could say something like: AGREEMENT AND DISCLOSURE STATEMENT SECURED BY DEED OF TRUST)
( A recent mortgage statement for your 2nd loan
Pay off of credit cards / auto loans:
