PRE-PRODUCTION SAMPLES:

Embroidered swatch or pre-production sample will be

provided FREE for each digitizing order. (One sample/design) All pre-prod.

samples will be shipped via UPS ground.

For pre-prod. samples, customer must provide merchandise. Each addt��l sample, $5

per location/design up to 7,000 stitches. Add $1.00 for each additional 1,000 stitches.

5 to 10 working days after receipte of written approval of pre-prod. Sample/

merchandise, whichever comes last. Delivery date may vary depending on Quantity

and Design.

12 colors. After 6 colors, please add $0.10 per piece for each additional color.

Add $0.15 per 1,000 stitches.

$0.15 per unit. Customer should provide the tags.

NO CANCELLATION ALLOWED ONCE ORDER IS PROCESSED. ALL PRICES ARE SUBJECT TO CHANGE WITHOUT NOTICE.

OTHER INSTRUCTIONS:

1. Orders requiring exact qty. of finished goods should INCLUDE 2% EXTRA/UNDER PIECES.

2. Quantity breaks are restricted to the same design, same item and same color seq. only.

3. Customer requesting for a specific thread brand/thread color may incur an addt��l charge.

4. All returned checks will be charged a $20.00 handling charge.

5. We will not take responsibility for merchandise sent to us which are damaged, wrong sized, colors etc... for contract embroidery.

6. Any merchandise received that needs to be separated and sorted by size will occur in a charge of $0.15 per piece.

* WE DO NOT COUNT MERCHANDISE ON CAPS OR FLATS. PLEASE ADD $ 0.15 MORE PER

PIECE IF COUNTING/SEPARATION IS NEEDED.

* Embroidery orders will ship 3-5 business days after date of written sample approval; includes receipt of all

merchandise.

* ALL Jacket orders, $4.00/pc Extra Charge in addition to the embroidery charge per embroidery location(s). Please see your

sales associate for details.

* Headmost limits liability for Jacket orders to the net price of the jacket up to a $40 maximum. A faxed/emailed copy of the

original invoice is required and will be applied to your account as an open credit only.

TERMS OF SALES

A. PAYMENT TERMS:

1. C.O.D. - We accept credit card, certified check and money order for new accounts with NO EXCEPTION. A

2% credit card handling charge applies to all CC transaction.

2. PREPAYMENT - Checks or money orders should be received before shipment. Prepayment is required when

merchandise is shipped to a different address on your request.

3. RETURNED CHECKS - All returned checks for any reason will have a $20.00 service charge.

B. PRICE:

1. Minimum order for shipping is 1 dozen. You may combine any style and color for large quantity price level.

2. All Caps/Visors shipped F.O.B. Ontario, California.

3. All prices are subject to change without notice.

4. Merchandise will be invoiced at prices in effect at time of shipping.

5. Sample or piece order: $5.00/Pc. plus shipping & C.O.D. charge.

C. RETURN POLICY:

1. You may for any reason return any or all Caps / Visors purchased within 15 DAYS of shipment received for full

refund of purchased price. Refund or credit on all returns exclude shipping freight.

All returns require our authorization in advance by phone.

2. The Caps / Visors must be received in original condition. PRINTED CAPS / VISORS OR PURCHASE OVER

15 DAYS ARE NOT RETURNABLE.

3. You pay the freight cost for returned shipment.

4. Any UNCLAIMED SHIPMENT will be charged a freight rehandling fee, $10.00 minimum.

5. Merchandise returned to us due to customer error will be subject to 15% restocking fee, $10.00 minimum.

D. DELIVERY:

1. We ship via UPS, unless otherwise instructed.

2. We ship in-stock Caps / Visors on the same day. However, order received after 4 p.m. (PST) will be shipped

on the next working day. All back orders confirmed before shipping.

3. Drop Ship: Please add $5.00 for each drop ship after first address.

E. OTHER:

1. A stocking fee will be charged for merchandise without a purchase order.

2. All Non-Headmost items will not be insured for shipping unless specified in the PO and a dollar amount specified.
