EMERGENCY PROCEDURES

In the event of an emergency, the security officer should notify the appropriate emergency personnel by calling emergency services and either the Cequr Security Area Coordinator, Vice President of Operations, or President in that order. The numbers are listed below:

Police/Fire/Smoke/Medical Emergenciesso

In the event of criminal activity, bomb threat, disturbances caused by unauthorized persons, call the Chicago Police at 911. If a report of fire or smoke has been confirmed, immediately notify the Chicago Fire Department at 911. Give your name (Security Officer_______), the address of the site is (Bell & Maypole or Wood & Maypole) and the kind of trouble you are reporting with a specific location.

Notify Cequr management as described in the chart above and advise of the situation.

In the event of a medical emergency, after contacting 911 certain facts will be required in order to prepare the paramedics for the situation. As a security officer and possibly the first to arrive on the scene, questions that will need to be answered are?

1. What is the victim’s name?

2. Is the victim conscious? Breathing? Responding?

3. What is the nature of the injury?

4. Is the victim wearing a medical ID?

5. Where is the victim now?

6. The approximate age of the victim?

7. The victim’s sex?

If the victim is unable to respond to your questions, locate the person who reported the emergency or the first person who is assisting with the situation in order to obtain this vital information. Time is critical in all medical emergencies!

At the conclusion of the situation, complete a detailed Incident Report. Be sure your report shows the time you notified emergency services, the time they arrived, and if applicable their names and badge numbers, and what happened. If the police file a report record the #. If the fire department responds record the Engine#.

Introduction

Something exciting is happening on Chicago’s Near West Side. As this historic part of the city experiences an incredible resurgence, a brand new neighborhood, Moreland Park, is now emerging.

This innovative, professionally planned community is unlike any other new development in the West Loop. With unbeatable prices, Moreland Park offers the buyer better value than can be found anywhere else. The West Loop is one of Chicago’s hottest neighborhoods, and the opportunity for appreciation is undeniable.

We at Cequr Security are fortunate to be included as part of the Moreland Lowrise Team. As the security contractor for security at Moreland Lowrise, we have been entrusted with the vital task of securing its buildings during construction.

Primary Goals:

1. Monitor entrance gates to deter unauthorized access

2. Prevent theft of copper and appliances

It is important the assigned security officers are fully familiar with the site and emergency procedures, so that they may better perform their functions as an aid to management, and as a protective service to the developers and contractors. The security service function at Moreland Lowrise is multi-faceted. Emphasis is placed heavily on alertness, awareness and presence, but also on appearance. This does not negate the need for security officers to act decisively and firmly when necessary.

All contractors and Moreland employees are to be served willingly and with courtesy in keeping with the Moreland Lowrise policy. All persons are to be addressed as Ms., Miss, Sir, or Mister. No person is to be addressed on a first name basis.

When Security Officers report for they will receive equipment necessary to the performance of their duties. This equipment will include keys, radio, and OTTS wand. All officers are to enter the condition of each item issued on his/her daily report.

Our security officers are very visible and therefore must be neat, clean and well groomed. As mentioned earlier, they must be courteous, well mannered and disciplined in attitude. Above all, the maintenance of a safe and secure environment at Moreland Lowrise is our primary goal.

To achieve these goals, it is necessary to remain active in patrolling, not congregating, and to avoid the temptation to visit in any area.

Schedule:

The security schedule for this property will be as follows:

Monday thru Friday: 3pm – 7am

Saturday and Sunday: 24 hours

General Duties

All assigned personnel will, at all times, conduct themselves in a professional and courteous manner to reflect credit upon the client, its management and guests.

All personnel shall report ready for duty, in full approved uniform, and shall be on time for their scheduled shift start. Officers shall not remain on facility premises after a scheduled shift has ended. No officer shall vacate an assigned post until properly relieved. If the post has no scheduled relief, such officers shall not leave until the shift has ended.

Officer’s responsibilities are a follows:

1. Security Officers shall perform their duties in an efficient, conscientious manner, and shall be courteous and helpful to all persons associated with Moreland Lowrise.

2. Comply with all City, State, and Federal laws as such may apply to the circumstances.

3. Know and conform to rules and regulations, as well as the Client policies, procedures and directives, and to immediately execute all lawful instructions given by a superior or Client representative.

4. Security Officers shall be responsible for continually maintaining the safety and security of the buildings during construction.

5. Security Officers are intended to serve as a visual and physical deterrent to crime at the site, respond to calls for assistance, and are to call for Police or other emergency response as circumstances warrant. If such crimes or violations of policy pose a threat of serious danger to the facility or persons in the facility, the security officer, may, in such an emergency, find it necessary to make an arrest. Such arrests must be in compliance with Illinois Statute.

6. Security Officers are intended to be the eyes and ears of Moreland Lowrise and ambassadors of goodwill.

7. Prepare and submit written Daily Security Reports of informative value, covering all activities during your shift of duty. All reports are to be PRINTED in BLACK ball point ink only and on provided forms.

8. All incidents, suspicious or unusual, will be reported to the appropriate authorities and documented by written Incident Report.

9. Security officers that will be required to make random rounds of the property at the minimum of every 30 minutes in order to insure the security of the site (Verified by Cequr’s Officer Tour Tracking System (OTTS)).

10. Keep all information confidential. No disclosure to workers or elsewhere off –site.

Procedures Regarding Daily Security Reports

DSR’s are meant to be more than records of when officers do rounds. They are used to look for patterns of activity across time that might indicate future problems and for anything that Cequr management or the client needs to know. In addition to communicating what was observed while on patrol, the reports provide important documentation in case the client claims that you as the security officer was responsible for damage or theft of property or any other type of negligence.

Because DSR’s are such an important part of your duties we have provided the following list of rules for completing your DSRs.

1. THERE IS NO SUCH THING AS TOO MUCH DETAIL!!! Use multiple sheets if necessary.

2. There IS such thing as not enough detail!!!

3. List all equipment that you receive at the beginning of duty and any malfunctions.

4. Include the time that you sign in ACCURATELY on the first line and the time you end the shift on the last line. Falsified times are grounds for immediate dismissal. If you are late it should be reflected on the DSR.

5. Complete each and every space as shown on the example DSR. Note: if you do not submit a DSR for your shift you will not be paid.

6. Note any important informative information:

1. Note all areas inspected and the time of inspection

2. What safety, fire hazards, property

3. Note all suspicious activity that you observe.

4. Make note of all persons loitering around the site.

5. Record the license plates of cars that are new to the area.

6. Record all visits by supervisors and or management.

7. Make note of any openings in fences or buildings that should be repaired or other safety, fire hazards, property damage or security violations.

8. Mechanical Problems

9. Make note of all unlocked gates and/or doors.

10. Make note of any lights that are out (street, parking lot, building, etc.).

7. THERE IS NO SUCH THING AS TOO MUCH DETAIL!!!

NO DSR NO PAY!!!

Procedures Regarding Incident Reports

Incident Reports must reflect ALL pertinent information regarding the reportable occurrence (i.e., who, what, when, where, why, how). Incident Reports must reflect time of occurrence, location, names of all victims and witnesses, nature of incident, beat and badge numbers of the first response team. Use a dictionary when writing your reports, your supervisors as well as the client are going to read the report and it is a reflection on you as an officer.

When confronted with a situation that calls for an Incident Report to be filed, an officer is to ALWAYS comply with the following procedure:

1. Contact the Area Coordinator or other management.

2. You are to give you name, the site at which you are assigned and the problem which is occurring.

3. If you cannot speak with the AC or management, refer to your Standard Operating Procedures when gathering information necessary to complete a full and detailed report pertaining to the incident.

4. When you contact the AC or management, relay the problem to him/her and get further instructions from him.

The above steps are to be followed in the order given in all circumstances requiring an Incident Report.

Uniform and Appearance

The following appearance and uniform policy are to be followed at all times while on duty. If a supervisor feels that an employee’s personal appearance is unacceptable, that employee may be asked to leave the workplace until properly dressed or groomed. Under such circumstances, the employee will not be compensated for his or her time away from work.

Appearance

All employees are expected to accept and follow, as a condition of employment, the standards listed below:

• Identification badges must be worn on the upper torso in a visible position with picture side out, at all times while on client/Cequr premises.

• Employees are expected to wear their complete Cequr uniform while on duty. The uniform is expected to be clean, neat, and pressed/ironed. Shoes are expected to be cleaned and shined.

• All fingernails should be cleaned and well groomed and of a length that does not hinder the employee’s ability to perform his or her assigned job duties.

• Canvas or athletic type shoes are not allowed.

• Mustaches and beards must be clean, well trimmed, and neat.

• Long hairstyles should be worn (with hair pulled back off the face and neck) to avoid interfering with job performance.

• Perfume, cologne, and aftershave lotion should be used moderately or avoided altogether, as some individuals may be sensitive to strong fragrances.

• Jewelry should not be functionally restrictive, dangerous to job performance or excessive (only two rings are allowed and only stud earrings, neck jewelry must be worn beneath the uniform).

• Facial jewelry, such as eyebrow rings, nose rings, lip rings, and tongue studs, is not professionally appropriate and must not be worn during business hours.

Uniform

The uniform at this site will consist of:

• Black BDU pants (See Employee Handbook for details)

• Black BDU Shirt or Cequr approved polo shirt (See Employee Handbook for details)

• White T-Shirt

• Black/Navy Blue Socks

• Black Leather Shoe (Athletic type shoes and patent leather are not allowed)

• Cequr issued coat or jacket

• Cequr approved hat

• Cequr ID

• Cequr approved 6-pt star patch

• Black name tape with white lettering for BDUs

• Nylon utility belt with 4 belt keepers

• Handcuff case with handcuffs (once certified)

The security officer performs activities necessary to the provision of a safe and secure environment for employees and visitors of the West Haven Construction Site.

Beginning of Shift

1. At the beginning of each shift report directly to the site.

2. Radio in your arrival to the Communication Hub (Peterson site) (10-41).

3. If you are relieving an officer find out any relevant information for you shift. Check Pass On Log for new information.

4. Check “Notices” sleeve of Standard Operating Procedure for new notices.

5. Begin DSR

6. Perform 1st Patrol.

DURING SHIFT

1. Make rounds of assigned areas, investigating all unusual or suspicious incidents and initiate corrective action as appropriate. There are 8 buildings/sites that officers are responsible for monitoring. See map for details.

1. There are 5 buildings under construction, Sales trailer, and McShane Contractor Trailer.

2. Officers should do rounds using the Officer Tour Tracking Service (OTTS) wand at random times at least every 30 minutes. There are 7 OTTS buttons located throughout the site. See map for details.

3. Officers should be aware of all persons walking with shopping carts. Civilians with shopping carts are notorious for breaking into construction sites to steal materials and carting them off. Times and descriptions of all persons with shopping carts should be noted on DSRs.

4. Check all gates to ensure that they are locked. If they are not locked secure them and note in the DSR.

5. If anyone requires access to the site while you are on duty:

1. Contact Cequr management via Nextel radio to get permission.

2. Request ID from the person requesting access.

3. If permission is granted, record what item(s) is being removed if any on your DSR, and record License Plate of vehicle or other relevant information.

6. NEVER leave the site without permission for ANY reason.

7. There should not be anyone other than the officer on duty in the squad car for ANY reason.

2. Communications

1. Officers MUST check in with the Communications Hub every 30 minutes. Failure to check in as scheduled will lead to disciplinary action up to and including termination.

3. Reporting and Confidentiality.

1. Accurately document daily activities on a Daily Security Report (DSR).

2. DSRs should be used to document anything that is noticed during your shift, NOT just the times that you do rounds (See example).

3. Complete all paperwork ACCURATELY as required (biweekly reports, Incident reports, days off request, etc.). NO DSR NO PAY!!! NO BIWEEKLY NO PAY!!!

END OF SHIFT

1. If you are being relieved by an officer:

1. Transfer possession of equipment to relieving officer and inform officer of events occurring during shift.

2. Remove any trash or garbage left behind in the squad car.

3. Radio your departure to the Communication Hub (10-42).

4. Depart site.

2. If you are not being relieved.

1. Radio your departure to Communications Hub (10-42).

2. Depart the site.

3. Return the Squad Car to the parking lot of the Cequr Office.

4. Return the Squad Car to the Cequr Office Building Parking Lot and not to the street. Be sure to park the car at the top of the parking lot to the right side away from the alley. Do not park the car in the middle of the lot. Anyone parking the squad car in the middle of the lot will be fined $50.00.

5. Place the “Club” on the steering wheel and arm the alarm.

6. Return the keys for the Squad Car to the slot in the door located to the right of the parking lot.

GENERAL NOTES

1. If your relief is more than 15 minutes late notify the AC or management via the Nextel Radio. NEVER leave the site before being relieved.

2. Do not run the engine of the squad unnecessarily. If we deem that you are using too much gas you will be charged $5/shift.

3. W. Sparkman has standing orders that state: “No officer shall be deemed relieved until the arriving relief officer has performed a physical check of all equipment that is used at West Haven site.” This includes the OTTS wand, the Nextel phone, the Nextel phone charger, and a Walk Around of the company car. Any damage or discrepancies should be noted in your DSR (Daily Security Reports). Damage to the vehicle must also be reported on a separate VIR (Vehicle Inspection Report).

4. Chicago Police Department car 65 is on patrol in the area. The cars phone number is 312-735-1761.

5. Keep the squad clean. Remove anything that you have placed in the squad at the end of your shift. Do not leave any type of food or drink in the squad.

6. Respond to requests for additional manpower to properly cover shift shortages.

TIMEKEEPING

Accurately recording time worked is the responsibility of every nonexempt employee. Federal and state laws require Cequr Security, LLC to keep an accurate record of time worked in order to calculate employee pay and benefits. Time worked is all the time actually spent on the job performing assigned duties.

Nonexempt employees should accurately record the time they begin and end their work, as well as the beginning and ending time of each meal period. They should also record the beginning and ending time of any split shift or departure from work for personal reasons. Overtime work must always be approved before it is performed.

Altering, falsifying, tampering with time records, tampering with VAS or barcode systems, or recording time on another employee’s time record is a criminal act and could result in disciplinary action, up to and including termination of employment and or prosecution, if it is determined it was done deliberately.

EMPLOYMENT TERMINATION

Termination of employment is an inevitable part of personnel activity within any organization, and many of the reasons for termination are routine. Below are examples of some of the most common circumstances under which employment is terminated:

RESIGNATION - employment termination initiated by an employee who chooses to leave the organization voluntarily, including no show or abandonment of job.

TERMINATION - employment termination initiated by the organization.

LAYOFF - involuntary employment termination initiated by the organization for nondisciplinary reasons.

MEDICAL TERMINATION - employment termination initiated by the employee or by the organization when an employee is unable, for health reasons, to continue to work.

RETIREMENT - voluntary retirement from active employment status initiated by the employee.

Cequr Security, LLC will generally schedule exit interviews at the time an employee resigns. The exit interview will afford an opportunity to discuss such issues as employee benefits, conversion privileges, repayment of outstanding debts to Cequr Security, LLC, or return of Cequr Security, LLC -owned property. Suggestions, complaints, and questions can also be voiced.

Since employment with Cequr Security, LLC is based on mutual consent, both the employee and Cequr Security, LLC have the right to terminate employment at will, with or without cause, at any time. Employee benefits will be affected by employment termination in the following manner. All accrued, vested benefits that are due and payable at termination will be paid.

EMPLOYEE CONDUCT

AND WORK RULES

To assure orderly operations and provide the best possible work environment, Cequr Security, LLC expects employees to follow rules of conduct that will protect the interests and safety of all employees and the organization.

It is not possible to list all the forms of behavior that are considered unacceptable in the workplace. The following are examples of infractions of rules of conduct that may result in disciplinary action, up to and including termination of employment.

• Theft or inappropriate removal or possession of property

• Falsification of timekeeping records

• Working under the influence of alcohol or illegal drugs

• Possession, distribution, sale, transfer, or use of alcohol or illegal drugs in the workplace, while on duty, or while operating employer-owned vehicles or equipment

• Fighting or threatening violence in the workplace

• Boisterous or disruptive activity in the workplace

• Negligence or improper conduct leading to damage of employer-owned or customer-owned property

• Insubordination or other disrespectful conduct

• Violation of safety or health rules

• Sexual or other unlawful harassment

• Possession of dangerous or unauthorized materials, such as explosives or firearms, in the workplace

• Excessive absenteeism or any absence without notice

• Unauthorized absence from work station during the workday

• Unauthorized use of telephones, mail system, or other employer-owned equipment

• Unauthorized disclosure of business “secrets” or confidential information

• Violation of personnel policies

• Unsatisfactory performance or conduct

Employment with Cequr Security, LLC is at the mutual consent of Cequr Security, LLC and the employee, and either party may terminate that relationship at any time, with or without cause, and with or without advance notice.

DISCIPLINARY PROCESS

For employees that are non-compliant, we have a progressive disciplinary process in place. The process is:

I. Non written verbal (informal conversation/alert)

II. Verbal warning (written)

III. 1st written warning

IV. 2nd written warning- (Team Leader notifies office/Walter Sparkman)

a. Walter Sparkman makes appointment for disciplinary report, having the employee come to the office. Based on the severity of the issue Walter Sparkman will determine whether suspension or termination is needed.

V. 3rd written warning termination (Walter Sparkman is notified)

Supervisors will submit all warnings to office within 24 hours. The second (2nd) written warning will be IMMEDIATELY brought to management’s attention. Disciplinary actions remain active/current for one year from last defense date. Any combination of six written offenses makes an employee subject to a disciplinary hearing. Please also note that depending on the severity of the infraction any given step or combination of steps may be skipped.

Each employee may request to have a disciplinary appeal with the management team after the first written warning for any offense.

ATTENDANCE AND PUNCTUALITY

UNIFORM PERSONNEL

To maintain a safe and productive work environment, Cequr Security, LLC expects employees to be reliable and to be punctual in reporting for scheduled work. Absenteeism and tardiness place a burden on other employees and on Cequr Security, LLC. In instances of tardiness, they shall notify their Team Leader, Area Coordinator, or Executive as soon as possible. In the rare instances when employees cannot avoid being late to work or are unable to work as scheduled, they are required to notify their Team Leader, Area Coordinator, or Executive no less than four (4) hours in advance of the anticipated absence. In the event a security officer is late with no call, no valid excuse, or calls off, he shall be subject to disciplinary action. If a security officer is a no show, no call, Cequr Security, LLC will consider that the officer has resigned his position with Cequr Security, LLC.

Please note that any officer that is more than 15 minutes late for a scheduled shift will be docked ½ hour pay. Any officer that is 30 minutes late shall be docked 1hours pay. Any officer that is later than 1 hour shall be docked 2 hours pay and be written up.

Poor attendance and excessive tardiness are disruptive. Either may lead to disciplinary action, up to and including termination of employment.

Please note that planned time off requires a 96 hour notification. Employees who call in must personally talk with the Team Leader/management, because “call ins” require verbal conformation from management, NO VOICEMAILS.

There is a limit of 3 call offs per 3 consecutive months. Call offs on holidays necessitates a doctors statement before the employee returns to work.

RETURN OF PROPERTY

Employees are responsible for all property, materials, or written information issued to them or in their possession or control. All Cequr Security, LLC property must be returned by employees on or before their last day of work. Where permitted by applicable laws, Cequr Security, LLC may withhold from the employee’s check or final paycheck the cost of any items that are not returned when required. Cequr Security, LLC will also take all legal action deemed appropriate to recover or protect its property.
