FUNCTION: Responsible for the administrative and fiscal operations of the Hospice. Directs the overall functions of the patient care teams within the Hospice... Responsible for defining, monitoring and evaluating quality patient care standards and outcomes for patient service within the teams. Based on internal and benchmarking data, directs improvements in efficiency and the development of new lines of service.

REPORTS TO: VP of Clinical Operations

RELATIONSHIPS: Develops key relationships with the Executive Team, Director of Clinical Services, Director Quality and Innovation Center, Director of Specialty Programs, Manager of Education, Associate VP for Operations, Director for Signature Care, and all VNS departments. All referral sources, health systems, vendors, physicians, patients and families and community partners, including the Leo Centerfor Caring at St. Ann’s Community.

HOURS: 40 hours/week

MANAGES/SUPERVISES: Hospice Team Members, Palliative Care Team, Nurse Practitioners, and Medical Director Hospice.

RESPONSIBILITIES:

1. Assumes twenty-four hour accountability for the supervision of all hospice staff. Ensures that all PHV staff competencies are completed as required by regulation and that staff adhere to agency standards. Responsible to initiate quality improvement plans with staff not meeting these standards.

2. Ensures that care provided by staff meets Federal, State and agency standards for cost effectiveness.

3. Develops and manages annual Hospice budget, including variable staffing plans. .

4. Analyzes available clinical and financial data to ensure cost effectiveness. Develops and monitors productivity standards. Continuously develops and implements care management approaches that meet or exceed national benchmarks for quality and cost.

5. Ensures to the maximum extent possible that services provided are reimbursed by third party payers. Collaborates with the Director of Quality to ensure quality standards are monitored and new regulatory changes are implemented.

6. Works collaboratively with the Executive Team to identify potential new services and/or enhancement of current program to meet the needs of customers (patients, health care team members, community providers, etc.)

7. Establishes and maintains effective working relationships with referral sources, Strong Health clinical leadership, and other major customers.

8. Monitors customer satisfaction with Hospice services and recommends and implements changes in operations as necessary.

9. In collaboration with the Director for Human Resources, assures that staffing levels are appropriate to meet business needs.

10. Monitors staff satisfaction and implements changes as necessary to assure that VNS is the employer of choice.

11. Actively participates in clinical research, including managing research related contracts, assessing study protocols and managing VNS patient and/or fiscal risks.

12 Assumes Hospice On-Call as assigned by rotation.

13. Responsible for the Key Performance Indicators  (KPI’s) established by their manager.

14. Other related duties as assigned.

QUALIFICATIONS:

1. RN license

2. Master’s degree in Nursing or Health related field.

3. Three to five years of clinical practice management experience.

4. Demonstrated ability to develop and manage effective teams.

5. Excellent verbal and written communication skills.

6. Excellent interpersonal and problem solving skills.

7. Knowledge of regulatory and accrediting agency requirements.

8. Ability to work with patients, families and staff of diverse backgrounds.
