The ZS is combined together and there is no way to determine what the amount of tonnage is found in a load by commodity. Where can I find the information needed in 17A, and what is meant by 17B?Answer: This information for 17A is the bidder’s information. It is meant by 17B to put an address if your billing address is different.How should the information be placed in lines 23 and 24? Do we just place the annual amount per year by adding up the 4 services requested for each individual year?Answer: Please complete B.4, Price/Cost Schedule and not 23 and 24.What do we do with the survey on pages 26 and 27, plus pages 32 and 33? Does the VA complete these forms, or does the VA send these surveys out to various references? Answer: Please read page 43 which provides instructions. Title of Project: Disposal of Solid Waste, Recyclables and Food Waste at the White River Junction, VT Veteran Affairs Medical Center.2. Scope of Work: Pick-up and disposal of all trash, recyclable materials and food waste generated at the White River Junction VT. VAMC these services are required at the Medical Center's main campus located in White River Junction, Vermont. Rental of 10 yard front load dumpster for trash collection. Dumpster to be located at the edge of parking lot between Buildings 6 and 7 and will include one (1) pick-up a week. Contractor will provide service to dispose of general waste from the 35 yard (government-owned) compactor to be located at the rear of B#31 and will include pick-up every 10-14 days (estimated 6-7 times per quarter) on a Tuesday. Contractor will provide service to disposal of all zero sorts / single stream recyclables from the 35 yard compactor (Government owned) located at the rear of B#31 which will include one (1) pick-up every other week. Contractor to provide service to pick-up cardboard, aluminum, tin containers, plastics #1 through -#7, magazines, glossy paper and glass containers for recycling. VA personnel will collect containers from throughout the facility and place in compactor. When applicable credit will be given to White River Junction VT VAMC, for the resale of cardboard and recyclable items. Government will receive the going rate for recyclables at time of resale or a credit. Credit will be given to White River Junction VT VAMC for the resale of recycled aluminum, tin, glossy paper and plastics. Government will receive the going rate for these recyclables at the time of resale.Contractor will provide15 clean, 48 gallon totes for the removal of food waste at the WRJMC. The pickup area is behind building 1 in the brown shed. Diverted organics will be picked up once a week on Mondays. Totes collected will be replaced on the dock with clean (empty) totes. All totes collected are counted as full totes. Volume will be recorded.3. Performance Period: April 1, 2018 through March 31, 2019 with 4 option years. 4. Type of Contract: Fixed-Price Contract for Services.5. Place of Performance: VA Medical Center 215 North Main St. White River Jct.Vt. 05009 and WRJ CBOC’s.B. CONTRACT AWARD MEETINGThe contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived.C. GENERAL REQUIREMENTS1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and together with associated sub milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed project management plan (PMP).2. All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms.3. Invoices provided monthlyD. QUALITY ASSURANCE SURVEILLANCE PLAN (QASP): 1. The ability to successfully provide the number and types of services required on a daily basis over an extended period of time requires a good Quality Assurance Surveillance Plan. The required services shall be delivered within the normal working hours (8A-4:30P), while meeting the quality standards required. The Contractor shall identify the solid waste, recyclables and food waste. The Contractor shall focus on delivering the primary services of collections, consolidation, transportation, shipping and disposal in a manner the meets the requirements. The Contractor shall identify the key staff members who will carry out the requirements. (see attachment D.3)E. ADMINISTRATION 1. Facility Orientation: An initial orientation of the facilities will be conducted by the COR at the start of the contract. The contractor shall be responsible for conducting orientation for new employees thereafter. 2. Accident Reporting: In the event an accident occurs on the Department of Veterans Affairs property or involving Government personnel or property, the contractor shall contact the VA Police immediately. A report shall be provided to the Contracting Officer and COR in writing that shall include the following: (1) the time and date of occurrence; (2) the place of occurrence; (3) a list of personnel directly involved; and (4) a narrative or description of the accident to include chronological order of the accident and circumstances; (5) corrective action to prevent future occurrences. 3. Locations: The government reserves the right to modify the contract as needed to add or delete VA Locations. 4.
