OVERVIEW

This contract will cover the requirements of Coffee via “Push Button” or “lever” machine for the Department of Human Services (DHS) / Danville State Hospital (DSH).

TERM OF CONTRACT

Anticipated Term of Contract: 01/01/18 (or upon full execution of contract; whichever is later) through 12/31/18

METHOD OF AWARD

The Commonwealth will award by lowest priced bid by a responsive and responsible bidder meeting the minimum qualifications/requirements set forth in this IFB.

Vendor must arrange a site visit and complete the attached site visit form to be considered responsive.

PRODUCT SPECIFICATIONS/REQUIREMENTS

An estimated quantity of 5 cases of Regular and 12 cases of decaffeinated each month for 12 months equaling a total of 204 cases is based on the following pack size: 2-2 liters or 4-1 liters/case mix ratio 35/1=39 gal/case (if vendor bidding different pack size or type of coffee being offered adjust to overall bid amounts/volume requested). Award will be made on cost per finished ounce of each type.

Dietary will call to arrange delivery date and amount needed each month in the event monthly case amount needs to be modified.

Delivery is to be made to the Dietary Dock.

100% pure coffee, Columbian and/or Arabica blend, decaffeinated AND caffeinated, Maxwell House brand or equivalent. Caffeine content: 97% caffeine-free if decaffeinated.

Product may be ‘bag in box’, closed type concentrated product/system, refrigerated/frozen only.

FREEZE-DRIED PRODUCT IS NOT ACCEPTABLE.

Shelf life: Refrigerated or thawed - 3 weeks

Frozen - 1 year.

If bidding differently, vendor to state ‘mix ratio’ to qualify cups per case of finished product. Mix ratio is: _________

EQUIPMENT SPECIFICATIONS/REQUIREMENTS

Product must meet the ‘push button’ or ‘lever’ machine dispensed specifications as outlined. Machine may not be dispensed in an open tap/non-portioned control flow.

Vendor shall supply water filters.

Successful bidder to furnish, install, and maintain at no additional cost to the buyer, all dispensing equipment as specified in bid during the term of this contract, to reconstitute the coffee at a mixing ratio and temperature to be determined by the Dietary Services. Equipment shall remain the property of the vendor and shall be returned at the end of the contract period when all purchased product is used. Vendor shall make arrangements and handle the return/pick-up of all equipment.

If machine and equipment are not picked up within 3 months following termination of contract and supply, hospital reserves the right to dispose of equipment.

Dispensing Equipment shall have: Metered portioned and continuous dispensing control; hot water dispensing available for alternate beverages, cleaning brushes, flex hose, and unit accessories as necessary. Locking capability for outside of unit. UL listed and N.S.F. approved.

Quantity of Dispensers requested: Two (2) each - Tray Service

Similar to Model Aromat Powerplex D4 or equivalent.

Style of Operation: portioned and continuous flow control, dispensing up to 180 degrees and as low as 160 degrees F.

Dispensing Style: Push button/lever/Portion Control, and Continuous Flow or level control.

INSTALLATION OF EQUIPMENT

Installation of dispensing equipment shall be arranged with the Dietary Services in advance of the contract period. Buyer shall provide utility connections above floor level and within six (6) feet of site location(s).

SERVICE: 24 hour, seven day emergency service shall be provided on all dispensing equipment, as well as a required preventative maintenance program, service checks, with reports:

Reports to begin:

- 1 at set-up and installation

-A preventative maintenance program service check report every two months thereafter, submitted to the Director of Dietary Services for file with Clerk Typist copy of contract

-Additional service reports for any emergency service calls in between and above required visits, to ensure the system is working properly.

**Note: Lack of required documentation will be cause for termination of this contract.

TRAINING: Vendor shall provide training on the operation and daily cleaning required for dispensing equipment to ensure uninterrupted coffee service.

CANCELLATION CLAUSE: Quantities listed are estimates only and may be increased or decreased in accordance with the actual requirements of the facility. With notification ten calendar days prior to delivery, the facility reserves the right to cancel any quantity for the order and agrees to pay the vendor only for products actually accepted by the department.

Vendor to specify brand, package size, mix ratio, cost, machine being supplied at time of bid.

NOTE: Dispensing machine shall be as listed below, or an approved equal that meets all the above metered portion controlled dispensing and other features as specified above:
