DOCUMENTATION CHECKLIST

1. Completed application – all attachments.

2. $550.00 for appraisal to be collected via debit/credit card. (Non Refundable Appraisal Fee). This fee will be collected after a fully executed purchase contract has been received.

3. Copy of Purchase Agreement or Completed Construction Bids with Cost Breakdown (new construction or Manufactured Home) plus plans/specs for home and foundation (for manufactured housing, foundation footings must be at least 4 feet deep or meet FHA minimum standards).

4. Copy of pay stubs covering last 30 days.

5. Copy of 2 years tax returns with W-2’s and/or 1099’s.

6. Copy of last 2 months bank/brokerage statements, all pages. (Cannot accept activity history)

7. Copy of Tribal Membership and Driver’s License and social security card.

8. Provide verification the accounts listed on the next page have been paid in full or brought current. Once the account is paid in full or brought current, contact the company in charge of that account and have them send you a letter. Make sure the letter makes reference to the specific account number in question. A copy of your credit report may be obtained by calling RELS at (877) 216-9150, speaking to a customer service representative and requesting your credit report. After verifying your identity RELS will mail you a copy. I apologize, but Wells Fargo Home Mortgage cannot provide a copy due to compliance and regulatory guidelines. (if necessary after a loan denial)

9. Provide a written explanation for all listed items. (if necessary)

10. Please provide 3 strong credit references. (if necessary) This can be the Cable Company, Electric Company, etc. Contact the company that can write a letter stating that you have made on time payments in the past.

LEASE LAND DOCUMENT REQUIREMENTS

11. Completed Land Status Form. This requires a signature of a tribal official.

12. Letter from Indian Health Service verifying their ability to install water/sewer –if applicable.

13. Completed Environmental Assessment Form – (the tribe completes.)

14. Copy of Certified Title Status Report (BIA Realty Office).

15. Copy of Deed to Restricted Indian Land (Individual-allotted Trust Land) or Tribal Lease Agreement (Homesite Lease-Tribal Lease Land) the lease agreement must be either a 50 year lease or a 25 year with a 25 year automatically renewable lease.
