Ward Attending Duties

Clinical Faculty Attending on Acute Care for the Elderly Inpatient Service. Previously also on Medicine Consult Service, Acute Stroke Team, and General Medicine Ward Service at University of Colorado Hospital. Provide bedside teaching, small group workshops, and formal didactics to medical students, interns, and residents rotating on service. As ward attending responsible for providing patient care for full panel of acutely ill inpatients and both supervising and teaching one intern, one resident, and one to two medical students. Specific Accomplishments

Quality Improvement Initiatives

Creation and Director of Acute Care for the Elderly Service at the University of Colorado Hospital

Demonstrated improved outcomes for vulnerable patients, improved patient and staff satisfaction. Developed data dashboard for quality assurance and drove quality improvement initiatives in readmissions, hand hygiene and in-hospital falls. Have expanded model from a single service to two clinical care teams and increased unit beds 47%. Multiple external institutions are interested in duplicating both our methods and outcomes. We have disseminated our quality processes and inter-professional leadership model at multiple national meetings including the Society of Hospital Medicine, American College of Physicians, and American Geriatrics Society. Examples of peer reviewed publications from ACE include educational, QI, and research in Med-ED Portal, the Journal of Nursing Care Quality, Neurohospitalist, and Journal of the American Geriatrics Society. Our ACE unit was the site for multiple QI projects including reducing medication reconciliation errors on admission and award winning work with discharge transitions. The unit is actively engaged in QI to improve patient satisfaction. Fall rates have dropped dramatically over the last two years representing a 38% reduction in fall rates on the unit. All cause 30-day readmissions have decreased from a baseline of 15.5% to 9.3%, representing a 40% reduction in all-cause readmissions. Hand hygiene rates have risen from 78% to sustained performance at greater than 97% adherence. The ACE methodology driving this accomplishment won the top innovation award at the 2013 National Society of Hospital Medicine Conference.

Improving Care for Stroke- Application of QI Principles to the Inpatient Neurologic Emergency.

Principle Investigator on a nationally award winning series of inter-professional comprehensive longitudinal quality improvement initiatives to reduce time to evaluation and treatment for ischemic stroke. The methodology employed represents a model for improvement of complex multi-departmental hospital processes with methodology and results published in peer reviewed publications. Tools and techniques employed to achieve these results disseminated through the National Stroke Association’s In-hospital Stroke Quality Improvement Initiative, for which I serve as Director, and the Society of Hospital Medicine Database of Quality Improvement.

Target Stroke Quality Improvement Initiative- Reducing door to needle time for thrombolysis in the ED.

Reduced time from triage to treatment from 71 minutes to 41 minutes. Exceeded goal of American Heart Association’s Target Stroke Program. Recognized at the 2013 International Stroke Conference for this accomplishment. Invited lecture on QI to reduce time to treatment at 2013 International Stroke Conference.

Acute Care for the Elderly- Geriatric Hospitalist Curriculum- 2006-Present

Development of Acute Care for the Elderly Services as an incubator for innovation in health care delivery for vulnerable elderly adults. It provides an educational curriculum to teach interdisciplinary care, continuous process improvement, and medical topics unique to the geriatric inpatient population. On-site Consultation by Dr. Brandon Koretz UCLA Geriatric Division Chief performed in 2008 through the Reynolds Foundation. Two novel small group workshops and external validation of competency-based examination from curriculum published by the American Association of Medical Colleges and the American Geriatrics Society. Published results demonstrating improvement in educational measures and quality measures for geriatric hospital care comparing ACE service to usual ward care in the Journal of Hospital Medicine.

Institute of Quality, Patient Safety, and Efficiency- 2012-Present

Core founding faculty for Institute covering the University of Colorado Hospital, Children’s Hospital, the School of Nursing, and the School of Medicine. Responsible for developing new quality improvement, patient safety, and leadership curriculum. This multi-year program is targeted at clinical medical director/nursing leader teams to develop a scope of leadership in the clinical microsystem which includes training on process improvement, financial impact of the value equation, effective teams, and management. Coach application of principles to multiple teams over sequential cohorts in this longitudinal training program.

Faculty for a national leadership course held annually at UCLA funded by the Reynolds Foundation. Pairs a two day intensive training program on healthcare finance, leadership, and change management with a 3 month post-course coaching program for participants to apply what they have learned to leadership challenges in their own institutions.

Transitions Project Director for Third Year Medical Students University of Colorado- 2008-2011

Project Director for The MSIII Medicine Clerkship Transitions Project designed to teach students how to recognize a patient at risk for failures at transitions and implement effective hospital discharge planning. Utilizes a multi-modal experiential and patient-based teaching strategy with didactics, small group workshops, and hands-on patient encounters. Provides choices for students to explore barriers to safe and efficacious transitions from the hospital employing home visits, follow-up phone calls, and participation in post-discharge clinic visits.

Medical Error Curriculum- Development and updates 2006-present.

Innovative longitudinal curriculum for Medical Error developed for Hospitalist Training Track Residents in the Internal Medicine Residency Program. This novel curriculum combining didactics on systems, heuristics, and disclosure with modified root cause analysis of actual adverse events has been published on Med-ED PORTAL a publication of the Association of American Medical Colleges. Continue to deliver course on disclosure of medical error.
