Moore Public Schools Athletic Department Eligibility Checklist

1) Pre-Participation (sports) physical A. Sports Physicals must be dated after May 1st of each year to be valid for the following school year. B. MPS offers low cost ($20) sports physicals in May of each school year. The 3 physical nights are as follows:

Night 1 - Monday, May 13, 2019 @ 6pm; Westmoore, Brink & West JH @ Westmoore High School

Night 2 - Thursday, May 16, 2019 @ 6pm; Southmoore, HWJH & Southridge JH @ Southmoore High School

Night 3 - Tuesday, May 21, 2019 @ 6pm; Moore, Central and HEJH @ Moore High School

C. All of the money collected from the MPS sports physicals is used to provide sports medicine supplies and equipment for MPS athletes. Sports physicals done at an urgent care clinic or a doctor's office do not provide supplies or equipment that is used to care for you/your athlete.

D. The sports physical consists of 2 pages: 1. Medical History Form - Must be completed online & printed out (instructions are below); must accompany the physician's evaluation form for physical night. 2. Physician's Evaluation Form - Must be completed by a physician, nurse practitioner, or PA; must be signed (printed name and signature) or stamped and dated by the certifying provider

2) Electronic Participation Forms: A. All athletic participation forms are online at RankOne. Please use the following web address to take you to the Moore Public Schools Online forms page:

B. The MPS Athletic Participation Packet & OSSAA Eligibility form must be completed online; there is also a fillable/ printable OSSAA Physical Form under the Download/Print tab: a. Participation Packet b. OSSAA Eligibility form c. Medical History form: print out, and complete the fillable PDF (OSSAA PHYSICAL FORM under DOWNLOAD & PRINT tab) and bring to physical night with your Physical Form.

3) Athletic Insurance: A. The Moore Board of Education does not assume responsibility for any athletic injuries to students. Any student participating in athletics must sign an insurance waiver B. Students who lack insurance coverage should strongly consider purchasing the policy made available from K and K Insurance at the following link:



4) Concussion Management: Athletes must complete a baseline exam every other year per the Moore Public Schools (MPS) Concussion Management policy. Dates for testing will be posted on the MPS Athletic Website, as well as on the individual school websites, and the RankOne calendar.

5) ALL ATHLETES MUST HAVE THEIR SPORTS PHYSICAL (MED HISTORY + PHYSICIAN'S EVAL), OSSAA ELIGIBILTY FORM AND MPS ATHLETIC PARTICIPATION PACKET COMPLETED PRIOR TO PARTICIPATING IN ANY ASPECT OF SPORTS AT MOORE PUBLIC
