| |Diabetes is a metabolic -disorder with inappropriate hyperglycemia either |

| | |

| |due to an absolute or relative deficiency of insulin secretion or reduction in |

| | |

| |the biologic effectiveness of insulin or both. It is also associated with disturbances |

| | |

| |concerned with protein, carbohydrate and lipid metabolism. The decreased uptake of |

| | |

| |glucose into muscle and adipose tissue leads to chronic extra cellular hyperglycemia |

| | |

| |which results in tissue damage and chronic vascular complications in both type 1 and 2 |

| | |

| |Diabetes Mellitus . [1] |

| | |

| |Hypertension affects approximately 70% of patients with diabetes and is |

| | |

| |approximately twice as common in persons with diabetes as in those without.[2] |

| | |

| |Hypertension in patients with diabetes causes a significant increase in the risk of |

| | |

| |vascular complications in this population.[2] |

| | |

| |The abnormalities in diabetic contribute to cellular events that causes atherosclerosis and |

| | |

| |subsequently increases the risk of the adverse cardiovascular events.[3] |

| | |

| |A study has demonstrated that elevated levels of hs-CRP are associated with |

| | |

| |increased cardiovascular risk.[4] |

| | |

| |The oxidative stress biomarkers Myeloperoxidase( MPO) powerfully enhances |

| | |

| |cardiovascular risk prediction.[5] |

| | |

| |Serum MDA level was significantly higher in longer duration of diabetes (>5 years) |

| | |

| |and presence of complications(both microvascular and macrovascular).[6] |

| | |

| |Risk of CVD increased significantly with increasing Total Cholesterol(TC) and Low |

| | |

| |Density Lipoproteins (LDL) Cholesterol.[7] |

| | |

| |6.2 REVIEW OF LITERATURE |

| |Amanullah et al. reported that in diabetic subjects hs-CRP level positively correlated with |

| |HbA1c.[1] |

| |Heslop et al. reported that measuring both MPO and CRP (C-reactive protein; a |

| |cardiac-related marker of inflammation) has added benefit for risk prediction |

| |of cardiovascular mortality than just measuring CRP alone.[5] |

| |Anand et al. reported that both hypertension and diabetes were proven as independently associated with an increased risk of CVD. |

| |Detection of hs-CRP should be given consideration while assessing cardiovascular risk in order to evaluate the risk of |

| |atherosclerotic vascular disease especially in patients with a hperlipidemia ,hypertension and diabetes to detect early CVD.[7] |

| |6.3 OBJECTIVE OF THE STUDY |

| |.To compare and to correlate the serum hs-CRP, Myeloperoxidase, Malondialdehyde,between patients with type 2 diabetes without |

| |hypertension, type 2 |

| |diabetes with hypertension and age and sex matched normal controls. |

| |.To correlate the levels hs-CRP, Myeloperoxidase, Malondialdehyde with glycemic control(HbA1c) between patients with type 2 |

| |diabetes without hypertension and type 2 diabetes with hypertension. |

| |.To correlate and to the levels of hs-CRP, Myeloperoxidase, and Malondialdehyde with Body Mass Index , waist hip ratio of the |

| |patient. |

| |.To correlate the levels of hs-CRP, Myeloperoxidase, MDA and lipid profile between patients with type 2 diabetes without |

| |hypertension and type 2 diabetes with hypertension. |

| | |

| |MATERIALS AND METHODS |

| |7.1 SOURCE OF DATA |

| |Individuals in the age group of 35-65 years who come to AJIMS hospital, Mangalore, |

| |diagnosed as diabetics (newly diagnosed or known diabetics on treatment) and diabetes |

| |with hypertension during the year 2011-2013 are considered as cases and are included in the |

| |study with age and sex matched controls. |

| |INCLUSION CRITERIA |

| |a)Age more than 35 years and less than 65 years. |

| |b) Known cases of type 2 diabetes mellitus on treatment and or newly diagnosed type 2 diabetes mellitus with and without |

| |hypertension. |
