Standing Orders - Hospice
Standing orders may be used by the Attending Physician as a part of the plan of care. These standing orders may be utilized in the event that one of the listed symptoms develops, at the discretion of the hospice nurse.
Standing orders shall be in compliance with all applicable state statutes and regulations and shall:
• Include the purpose or conditions under which the standing order will be implemented, (Example - Purpose: Nausea; Condition - Compazine 10mg po q 12 hrs. If nausea uncontrolled by Compazine, may use Tigan 250 mg q 6 hrs prn.)
• Be drug, treatment or procedure specific and not allow for non-prescriber’s choice,
• Be individualized, signed and dated by the prescriber and included in the patient’s record.
Once a standing order has been implemented, the hospice nurse shall write an interim order detailing this change, utilizing Agency guidelines for interim orders and timeliness. The route of patient’s medications can not be changed without consulting physician.
Standing order content shall be reviewed and approved by the Medical Director at least annually.
