Instructions for Completing the Financial Status Report

Basic Grant to States, CFDA 084.048A and Tech-Prep CFDA 084.243A

A separate set of Financial Status Report (FSR) forms are to be used for each Federal Funding Period as reported in Block 7, Form A.

Instructions for completing Form A:

Block Instructions

1. Grant recipient submitting the report.

2. PR/Award numbers as indicated in Block 5 of the Grant Award Notifications for the Basic Grant to States and Tech-Prep Education.

3. For optional use by those agencies needing cross reference identification.

4. Check YES if this is the final report for a grant award and there are no amounts reported in column h on Form B (unliquidated obligations). The report is final when all accrued expenditures (obligations) have been expensed (liquidated). FSRs that are not final are considered interim reports. Check NO if this is an interim report.

5. Identify the accounting basis used by the Grantee. If the modified accrual basis is used, it should be so indicated by adding the word “modified” in this block.

6. Enter Federal Funding Period based on information obtained in Block 6 of the Grant Award Notification.

7. If this is an interim report, specify the Federal Funding Period as requested in Block 6 of this form. If this is a final report, specify the 27 months (Federal Funding Period plus 12 months of Carryover) of accrual activity, i.e., July 1, 1999 through September 30, 2001. DO NOT include time periods where liquidations or other adjustments were made.

8. Include any remarks that are necessary to explain any specifics in the report. Attach additional information if needed.

9. The report must be certified by the Executive Officer, or designee, of the Grant recipient, as appropriate.

Instructions for completing Form B:

Secondary/Postsecondary – This form has been amended to consolidate the reporting of secondary and postsecondary outlays. In states where separate agencies have responsibility for reporting and certifying the expenditure of funds, the grantee agency is responsible for aggregating the data and submitting it to the Division of Vocational-Technical Education. Separate reports from sub-grantees will not be accepted.

Column Instructions

a On an interim FSR, this column must be zero. On the final FSR, list here what was reported in column e on the interim FSR.

b On an interim FSR, report total outlays, including State and local matching where required, for the Federal Funding Period. On the final FSR, report the outlays during the carryover period (between the 15th and 27th month).

c On an interim FSR, report program income credits, for the Federal Funding Period. On the final FSR, report the credits during the carryover period (between the 15th and 27th month).

f This column must include all non-federal outlays reported in Column e.

g Column e minus Column f.

h Applicable to interim reports only, enter the Federal share of unliquidated obligations as of the end of the Federal Funding Period.

For Line 2, enter the amount set aside for State Administration activities.

For Line 5, enter the amount set aside for State Leadership program activities.

For Line 10, enter the amount set aside for the Single Parents, Displaced Homemakers and Single Pregnant Women programs activities.

For Line 11, enter the amount set aside for the Sex Equity Program activities.

For Line 12, enter the amount set aside for the Criminal Offender Program activities.

For Line 13, enter the amount set aside for subrecipient grantees, under Title II, Part C.

For Line 22, enter the amount awarded for the Tech-Prep grant (V243A…) in Block 7 of its Grant Award Notification.

Line Instructions

1. Report the total outlays from Title II for the Basic Grant to States (the sum of lines 2, 5, 10, 11, 12 and 13).

2. Report the total for lines 3 and 4.

3. Report the outlays for the State Administration activities as described in PL 101-392 §102(a)(4)(B).

4 Report the outlays for the Sex Equity Coordinator as described in PL 101-392 §102(a)(4)(A).

5 Report the total outlays for State Programs from Title II, Part A as described in PL 101-392 §102(a)(3) (the sum of lines 6 through 9).

6 Report the outlays for State Programs and Leadership as described in PL 101-392 §201(c).

7 Report the outlays for professional development activities as described in PL 101-392 §201(b)(1).

8 Report the outlays for curriculum development as described in PL 101-392 §201(b)(2).

9 Report the outlays for assessment as described in PL 101-392 §201(b)(3).

10 Report the outlays for serving single parents, displaced homemakers, and single pregnant women under Title II as described in PL 101-392 §102(a)(2)(A).

11 Report the outlays from Title II for the Sex Equity program described in PL 101-392 §102(a)(2)(B).

12 Report the outlays for programs in correctional institutions from Title II, Part B as described in PL 101-392 §102(a)(5).

13 Report the total outlays for Title II, Part C for Basic Programs to eligible recipients described in PL 101-392 §102(a)(1) (sum of lines 14 through 21).

14 Report the outlays for apprenticeship programs described in PL 101-392 § 235(c)(2)(J).

15 Report the outlays for upgrading of curriculum described in PL 101-392 § 235(c)(2)(A).

16 Report the outlays for equipment purchases described in PL 101-392 § 235(c)(2)(B).

17 Report the outlays for inservice training for integration as described in PL 101-392 §235(c)(2)(C).

18 Report the outlays from Title II for guidance and counseling described in PL 101-392 §235(c)(2)(D).

19 Report the outlays for tech-prep education described at § 235(c) (2)(G).

20 Report the outlays for adaptation of equipment, supplementary services, and special populations coordinators as described in PL 101-392 § 235(c)(2)(F)(H), and (I).

21 Report all other outlays described in PL 101-392 §235, which are not included in lines 14 through 20.

22 Report all outlays for the Tech Prep Education Grant (V243A…) as described in PL 101-392, Title III, Part E.
