This list will help you meet the requirements to get an enhanced driver license (EDL) or enhanced identiication card (EID). We can only accept original documents or government-certiied copies with a raised seal or stamp. We cannot accept internet printouts, temporary, uncertiied, notarized, or altered documents. We may retain copies of any documents you bring.

If you have a Washington driver license or identification card with a photo, you will need to bring the following when you go to a driver licensing ofice: 1. Proof of United States citizenship 2. Social Security number

If you do not have a Washington driver license or identification card with a photo, bring: 1. Proof of United States citizenship 2. Social Security number 3. Proof of identify 4. Proof of Washington State residence

1Proof of United States citizenship To prove citizenship, show us of one of the following:

? United States Passport that is valid or has been expired no more than ive years

? Valid United States Passport card

? Original certiied* state birth certiicate (issued by the city, county, or state)

? Certiicate of naturalization

? Certiicate of citizenship

? Department of State Consular Report of Birth Abroad

? Valid out-of-state enhanced license or ID

* A certiied birth certiicate has a registrar's raised, embossed, impressed, or multicolored seal; a registrar's signature; and the date it was iled with the registrar's ofice, which must be within one year of your birth.

2Social Security number Be prepared to tell us your Social Security number. You don't need to show us the card. Your name and Social Security number must match Social Security records.

3Proof of identity Show one of the following:

? Valid United States Passport

? Valid state-issued driver license, enhanced driver license, or ID card

? Military ID

? Government employee ID card (state, federal, or city) showing your name and signature

? Tribal identiication card

4Proof of Washington State residence To prove you live in Washington, show us one document from this list with your name and current Washington resident address.

? Concealed weapons permit

? Homeowner's insurance policy or invoice showing the address of the insured property or residence

? Home utility bill (such as gas, electricity, water, garbage, or land line telephone, or cable) or hook-up work order dated within the past 60 days. You must be the only person named on the bill or work order.

? Mortgage documents

? Property tax bill or statement dated within the past 12 months

? Your name and address in a current phone book made by a telephone book publisher

? Moorage bill or contract showing that you live on a boat in a marina

? Federal or state government agency-issued check

? Tribal ID that contains your current resident address

? A iled property deed or title for your current residence

? A document from a public or private institution (hospital, college, Microsoft, etc.), or a government agency (Veteran's Administration, corrections, local housing authority, etc.) that contains your current resident address

? A document from a senior housing, retirement home, or assisted living facility on company letterhead with a telephone number that could be used for veriication of your residency there

? Out-of-state license with a Washington State address. A change of address sticker or handwritten address on the license is not acceptable.

If you don't have any of those residence documents, you can show us TWO documents from this list with your name and current Washington State residence address:

? Auto insurance policy (not the proof of insurance card)

? Vehicle Title

? Washington State business license

? Current Washington State voter card

? Home utility bill (such as gas, electricity, water, garbage, or land line telephone, or cable) or hook-up work order dated within the past 60 days. Two or more unrelated people may be named on the bill or work order.

? Pay stub that contains your name, your current residence address, your employer's name, and your employer's phone number of address

? Business mail dated within the last 60 days. Mail must include your irst and last name, and must be from state or federal revenue departments, the Social Security Administration, the U.S. Treasury, or the IRS. It cannot be addressed "in care of," "for," or "parent of."

? Medical record paid by insurance or a medical bill

? Medicaid card or DSHS medical coupon

? Professional license (nurse, physician, engineer, etc.)

? Selective service card showing a Washington State address

? Professionally-iled tax return or iled copy sent to you by the IRS for the most current tax iling year

? Transcript or report card for the current school year from an educational institution in Washington State

? W-2 form from the previous year

? Earnings and Deducation Statement from a Washington State agency

You can use documents that are in your spouse's name if your last name and that of your spouse are the same or if you bring in acceptable connecting documents (for example: a marriage certiicate). Your spouse does not need to be present and does not need to sign a declaration.

*Proof of name change If you have a valid driver license, ID card, or EDL/EID and you want an EDL/EID displaying your current name, you must show one of the following:

? A marriage certiicate (including same-sex marriage certiicate) that has a control or ile number and has been iled with the county or authorized issuing authority. Church or wedding chapel forms are not acceptable.

? A court order iled and certiied by the court

? A divorce decree showing the new name or authorizing a name change, iled and certiied by the court

*Minor (under age 18) If you are under 18, you may use your parent's or legal guardian's residence address as proof of residence. Your parent or legal guardian must live in Washington State and must show us acceptable documents proving their identity, residence and relationship to you.

If you are under 18, you may establish your identity with a parent or legal guardian afidavit. When you apply, your parent or legal guardian must come with you to the licensing ofice, sign the afidavit, and show us at least one acceptable document to prove their identity and one other document to prove they are your parent or legal guardian. If your parent or legal guardian's proof of identiication is an out-of-state driver license or ID card, or if your last name differs from that of your parent or legal guardian, we will require more documents.
