a. 4345 S. Packers- Press Brake Department 40- On or about September 23,2014, provisions were not made to prevent unintended operation (actuation) of a foot treadle of Milwaukee Press brake# 7, Serial #171, due to falling or moving objects or by accidently stepping on the treadle.

b. 4400 S. Packers- Department 7- On or about September 23,2014, provisions were not made to prevent unintended operation (actuation) of a foot treadle of Verson Press brake # 20, Serial # 1968, due to falling or moving objects or by accidently stepping on the treadle.

c. 4400 S. Packers- Department 7- On or about September 23,2014, provisions were not made to prevent unintended operation (actuation) of a foot treadle ofVerson Press brake# 22, Serial# 23467, due to falling or moving objects or by accidently stepping on the treadle.

e. 4400 S. Packers - Depmiment 7 - On or about September 23, 2014, provisions were not made to prevent unintended operation (actuation) of a foot treadle ofVerson Press brake# 26, Serial# 23480, due to falling or moving objects or by accidently stepping on the treadle.

Among other methods, one feasible abatement method to correct this hazard would be to follow the American National Standards Institute ANSI B11.3-2012, Section 6.10.2 for machine tools- Safety Requirements for power press brakes, Section 6.1 0.3, by ensuring a protective covering over the treadle is provided to prevent unattended machine actuation.
