About this recall

Your safety, health and well-being are important to us. We want to let you know that on August 27, 2018, Accord Healthcare Inc. issued a voluntary, nationwide recall of one lot (or batch) of 100-count hydrochlorothiazide 12.5 mg tablets.

Why this medication was recalled

This medication was recalled because of a tablet mix-up. A pharmacy found that bottles for this one lot of 100-count hydrochlorothiazide 12.5 mg tablets actually contained 100 tablets of a different medication called spironolactone 25 mg tablets. It's important to know that to the best of our knowledge, as of the date of this notice, Accord Healthcare, Inc. hasn't received any complaints or information about medical illness or harmful effects caused by the recalled medication.

As of August 27, 2018, this is the only lot number affected by this recall:

What these medications are used to treat

Both hydrochlorothiazide and spironolactone are used to treat high blood pressure.

Steps you can take to find out if your medication was recalled

1. Check the lot number on the label of your pill bottle. Look for a number that's either on the label printed by the pharmacy or stamped onto the bottle (or package) itself. The number may or may not be listed as "LOT." If you can't find this information, please call the pharmacy you used to fill your prescription. > If your lot number is PW05264, your medication was recalled. > If your lot number is something different, your medication wasn't recalled.

2. Look at the pills in your bottle. Hydrochlorothiazide 12.5 mg tablets are light orange to peach in color, and are round, biconvex tablets with an "H" on one side and a "1" on the other side. We've included an image below so you know what they should look like. If the pills in your bottle look different, your medication may be affected by the recall. You can always contact your pharmacy or doctor if you're not sure what the pills should look like.
