You have earned the right to certain veteran’s benefits. Use this form to make it easier for your family in case of an emergency. Make sure to retain a copy for your records, including one in a safe deposit box. If possible, attach a copy of your veteran’s discharge/separation papers. In order to have a record of this information to establish eligibility for veterans and widow benefits in the future, it is suggested a copy be furnished the Veterans Service Agency in your area. Plus, a copy of your discharge and this document should be filed in your County Clerk’s office.

If your or your spouse had a previous marriage, it is important you list how these marriages ended (death, divorce, and annulment) and you should list the dates and places of termination, as the VA needs marriage legality proof.

National Guard & Reserve Eligibility

The National Guard and Reserves are required to meet the same eligibility requirements as the full time federal active veterans.

To be eligible, the National Guardsman/Reservist must have served at least one day on federal active duty during any of the delimiting periods as set forth in Article IV, Section 1 of The American Legion National Constitution. The person must either have an honorable discharge or currently be serving either in the Guard/Reserve or on federal active duty.

The key to determining if a Guardsman/Reservist has been on or is currently serving on federal active duty is the “Authority Line” on the activation orders of the Guardsman/Reservist.

In both cases “Title 10, Subsection 672 or 12301” are orders from the Secretary of Defense and are federal orders.

The authority the Governor uses to activate the National Guard as an individual or unit is “Title 32” orders, i.e. Weekend Drills and Annual Training. These are not federal orders. The Reserves have similar reserve orders which are “Title 10, Subsection 270.” This authority code gives the reserve component the authority to activate the reserves for Weekend Drills and Annual Training.

A DD-214 will be issued for the time on federal active duty or a DA-1059 for a completion of a school will be issued with a character type of discharge. All Reserve components send their service members to “Basic Training” using Title 10, Subsection 672/12301 orders.

Throughout the year, there are times your Post may be contacted concerning Membership Renewal, DISPATCH subscriptions, Awards, Veterans Affairs information or on other important matters.

1) 2003 dues amount, and effective date of change.

2) Address to which the member mails the dues.

3) Permanent address of the Post; this could differ from the mailing address.

The information reported for dues mailing purposes (Nos. 1 & 2 above) will be printed by National Headquarters on dues notices mailed to members. If at anytime during the year, there is a change in Post dues or the mailing address where members mail their dues, the Post should notify the Department Headquarters immediately. The Department will then report the changes to National.

Please make sure the data provided is correct and is submitted to your Department before April 15, 2002. For the first renewal mailing, no change in the amount of dues or remittance address can be made after this deadline. Other deadline dates will affect later mailings.

Requests for the issuance or replacement of military service medals, decorations, and awards should be directed to the specific branch of the military in which the veteran served.

However, for Air Force (including Army Air Corps) and Army personnel, the National Personnel Records Center will verify the awards to which a veteran is entitled and forward the request with the verification to the appropriate service department for issuance of the medals.

The Standard Form (SF 180), Request Pertaining to Military Records, is recommended for requesting medals and awards. Provide as much information as possible and send the form to the appropriate address from the following table:

Cold War Recognition Certificate

In accordance with section 1084 of the Fiscal Year 1998 National Defense Authorization Act,

the Secretary of Defense approved awarding Cold War Recognition Certificates to all members

of the armed forces and qualified federal government civilian personnel who faithfully and

honorably served the United States anytime during the Cold War era, which is defined as Sept. 2,

1945 to Dec. 26, 1991.

The only official website on which to request certificates is noted below. The site is operated by the United States Army, the executive agency for the Cold War Recognition Program. Cold War Recognition Certificates are available to qualified individuals at no cost. Any other site offering these certificates or replicas for sale or purchase are not official sites and are not approved or endorsed by the US Army.

Due to the remarkable success of this program, turn-around time for mailing certificates will be a minimum of 6 months. The CWRS Operations Team is working as fast as possible to clear the backlog. Please do not request feedback prior to 6 months from the request date; your patience and interest in the Cold War Recognition program is appreciated.

Who is eligible? All members of the armed forces and federal government civilian personnel who faithfully served the United States during the Cold War era, Sept. 2, 1945 to Dec. 26, 1991. Individuals requesting a certificate will certify that their character of service was honorable. Acceptable supporting document for proof of service is any official government or military document with recipient's name, Social Security Number or Military Service Number or Foreign Service Number, and date of service.
