| |6.1 |Need for the Study | |

| | | |Asthma is defined as a chronic inflammatory disease of airways that is |
| | | |characterized by increased responsiveness of the tracheobronchial tree |
| | | |to a multiplicity of stimuli. It is manifested physiologically by a |
| | | |widespread narrowing of the air passages. |
| | | | |
| | | |Clinically manifested by paroxysms of dyspnea, cough, and wheezing. |
| | | |Asthma is an episodic disease, with acute exacerbations interspersed |
| | | |with symptom-free periods. |
| | | | |
| | | | |
| | | |The prevalence of asthma is rising in many parts of the world. 300 |
| | | |million people worldwide suffer from this disease. (5-2% of |
| | | |population).WHO estimate is that there are 15 to 20 million people with|
| | | |asthma in India and effects 7% of population. |
| | | | |
| | | |Bronchial asthma occurs at all ages but predominantly in early |
| | | |life.About one-half of cases develop before age 10, and another third |
| | | |occur before age 40. In childhood, there is a 2:1 male/female |
| | | |preponderance, but the sex ratio equalizes by age 30. Some 4–5% of |
| | | |adults and up to 10% of children are estimated to experience episodes |
| | | |of asthma. |
| | | | |
| | | |An absolute 2% increase in the prevalence of asthma would result in |
| | | |additional 20 million people with disease in india by 2050. |
| | | | |
| | | |In conventional mode of treatment, the available agents for treating |
| | | |asthma are those that inhibit smooth-muscle contraction |
| | | |(quick relief medications) and agents that prevent inflammation |
| | | |(long-term control medications).These agent’s produce drug dependence |
| | | |and side effects. In addition there are drugs which induce asthma |
| | | |(NSAID’S). |
| | | | |
| | | |For years I was puzzled with the management of asthma.When a person |
| | | |came to me and asked: "Doctor, can you cure asthma?" I would say, "No."|
| | | |But now I am beginning to get quite liberal on asthma, since I have |
| | | |learned that asthma is a sycotic disease, and since I have made |
| | | |judicious application of anti-sycotics I have been able to relieve or |
| | | |cure a great number of such cases. |
| | | | |
| | | |You will find in the history of medicine that wherever asthma was |
| | | |cured, it has been by anti-sycotic remedies. That is one of the first |
| | | |things I observed, that outside of sycotics you will seldom find a cure|
| | | |for asthma. |
| | | | |
| | | |Asthma, when hereditary, is one of the sycotic complaints of Hahnemann.|
| | | |- I have cured a very large number of such cases of asthma |
| | | | |
| | | |Asthma in children of sycotic parents (Nat. s.).Several cases of asthma|
| | | |have been cured by this remedy. In fact, Natrum sulphuricum is one of |
| | | |the best, one of the clear-cut indicated remedies for those |
| | | |constitutional conditions in children that result in chest catarrhs and|
| | | |asthmatic complaints. |
| | | | |
| | | |When the chest is filling up with mucus, rattling of mucus, |
| | | |expectoration of large quantities of white mucus, with asthmatic |
| | | |breathing in young subjects, this remedy must be thought of. |
| | | | |
| | | |Due to rapid industrialization and urbanization through out region,the |
| | | |prevalence of asthma is predicted to increase rapidly in coming year. |
| | | | |
| | | |This area being highly polluted due to dust and number of cement |
| | | |factories, vehicles etc the number of asthma cases are increasing day |
| | | |by day. |
| | | | |
| | | |From various homoeopathic literatures and reports it is evident that |
| | | |homoeopathic treatment is very effective in treating bronchial asthma. |
| | | | |
| | | |Hence this study “STUDY ON BRONCHIAL ASTHMA” to evolve a suitable |
| | | |homoeopathic approach in the effective management of bronchial asthma |
| | | | |
| |6.2 |REVIEW OF LITERATURE |BRONCHIAL ASTHMA-DEFINITION – |
| | | |Increased responsiveness of lower airways to multiple stimuli; |
| | | |episodic, and with reversible obstruction; may range in severity from |
| | | |mild without limitation of patients activity to severe and |
| | | |life-threatening. Severe obstruction persisting for days or weeks is |
| | | |known as status asthmaticus. |
| | | | |
| | | |EPIDEMIOLOGY AND ETIOLOGY - Some 4–5% of adults and up to 10% of |
| | | |children are estimated to experience episodes of asthma. Basic |
| | | |abnormality |
| | | |is airway hyperresponsiveness to both specific and nonspecific |
| | | |stimuli.. |
| | | | |
| | | |PATHOGENESIS - Common denominator underlying the asthmatic diathesis is|
| | | |nonspecific hyperirritability of the tracheobronchial tree. Airway |
| | | |reactivity may be increased by a number of factors: |
| | | |Allergenic (pollen, house dust) |
| | | |Pharmacologic(drug induced particularly NSAIDS), |
| | | |Environmental(cold air), |
| | | |Occupational,infectious, |
| | | |Exercise-related |
| | | |Emotional. |
| | | | |
| | | |Types of asthma-there are two types of asthma, |
| | | | |
| | | |Extrinsic asthma(allergic) |
| | | |Intrinsic asthma(idiosyncratic) |
| | | | |
| | | |Extrinsic asthma-This entity has also been termed atopic asthma or |
| | | |IgE-mediated asthma. |
| | | |There is hereditary disposition. |
| | | |Intrinsic asthma-This is also known as idiopathic or cryptogenic |
| | | |asthma. There is no family or personal history of atopic disease. |
| | | | |
| | | |Clinical features |
| | | |The classic symptoms of asthma are intermittent, reversible attacks of |
| | | |dyspnoea, wheezing and cough, alternating with periods of remission. |
| | | |Some patients may present with vague symptoms like heaviness or |
| | | |tightness in the chest or bouts of cough. |
| | | | |
| | | |Wheezing may be audible |

| | | |General management- |
| | | |Avoid the allergen you are sensitive to. |
| | | |Do regular light exercise like brisk walking or jogging. Heavy exercise|
| | | |can precipitate an attack of asthma, so always do mild exercise without|
| | | |putting too much strain on your body. |
| | | |Learn breathing exercises to improve your lung capacity. Of especial |
| | | |use is ‘pranayam’, a yoga exercise |
| | | |Learn some stress relieving exercises, meditation, or yoga to minimize |
| | | |the psychological factors related to disease. |
| | | |Eat healthy, nourishing and well balanced diet. |

| | | |Useful to the physician in assisting him to cure are the particulars of|
| | | |the most probable exciting cause of the acute disease ,as also the most|
| | | |significant points in the whole history of the chronic disease, to |
| | | |enable him to discover it’s fundamental cause,which is generally due to|
| | | |chronic miasm.In these investigations,the ascertainable physical |
| | | |constitution of the patient(especially when the disease is chronic),his|
| | | |moral and intellectual character,his social and domestic relations,his |
| | | |age,sexual function etc,are to be taken into consideration. |

| | | |Miasm’s- The monstrous internal chronic miasm - the psora, the only |
| | | |real fundamental cause and producer of all the other numerous, I may |
| | | |say innumerable, forms of disease, (1) which, under the names of |
| | | |nervous debility, asthma and ulceration of the lungs, and pains of |
| | | |thousands of kinds, ..etc., figure in systematic works on pathology as |
| | | |peculiar, independent disease. |

| | | |Dr. J. Grosso of Argentina, referring to the miasmatic remedy, |
| | | |states-When we say 'miasms' we mean 'causes', the etiology of acute and|
| | | |chronic diseases. Thus, we talk about a psoric-sycotic asthmatic |
| | | |syndrome |
| | | |Asthmatic conditions seem to call for ARSENIC, the constitutionality |
| | | |is not reached by ARSENIC, its fundamental symptoms are not similar. In|
| | | |syphilis and psora ARSENIC acts a long time and eradicates the |
| | | |complaints, when similar to them, but it is not similar to sycosis; |
| | | |ARSENIC does not go to the bottom of the trouble, but THUJA and NATRUM |
| | | |SULPH. will take up the work and cure. NATRUM SULPH and THUJA bring the|
| | | |primitive manifestation that has been suppressed for years." |

| | | |Therapeutics- |
| | | |Ars alb has been found to cover nearly 50-60% cases of actual asthmatic|
| | | |attacks.some cases where Ars alb seems indicated but the patients are |
| | | |agg by warmth Ars iod is helpful.He also mentions that Nat sul will |
| | | |have to be given in order to cure cases of asthma, other medicines |
| | | |found useful in asthma are Spongia, Kali ars, Kali bich, Medorrhinum, |
| | | |Ipecac, Nat ars., Pulsatilla, Nux vom etc. |

| | | |Ars alb-The time of attacks is just after midnight. The patient has a |
| | | |great deal of anguish and restlessness; he cannot lie down for fear of |
| | | |suffocation. There is anxiety and general sweat, and if the patient |
| | | |drowses off he is awakened with burning pain and soreness in the chest |
| | | | |
| | | |Natrum sulph-Its general symptoms are worse on change to damp weather. |
| | | |Its symptoms are moist asthma, with a great deal of rattling in the |
| | | |chest. The symptom of looseness of the bowels after each attack has |
| | | |been repeatedly verified; i. If symptoms indicating a sycotic taint be |
| | | |present, it will be all the more strongly indicated. The attacks |
| | | |generally come on about 4 or 5 o'clock in the morning with cough and |
| | | |raising of glairy slime; expectoration greenish and copious. The asthma|
| | | |of hay fever finds one of its remedies in Natrum sulphuricum. |
| | | | |
| | | |Nux vomica-Is a useful remedy when the asthmatic attacks are brought on|
| | | |by gastric disturbances; simple spasmodic asthmas; there is some relief|
| | | |by belching, the patient must loosen the clothing. It must also be |
| | | |thought of in those who drink much coffee or liquor. There is a |
| | | |constricted feeling at the lower part of the chest. Where there is much|
| | | |abdominal irritation present with much flatulence.Patients are highly |
| | | |irritable. |

| | | |Pulsatilla - Asthma, which comes on or is worse in the evening, and is |
| | | |accompanied with constant chilliness, dizziness when rising from a |
| | | |seat, nausea and vomiting of mucus, palpitation of the heart, nightly |
| | | |diarrhoea and deranged menstruation, is immediately relieved by this |
| | | |medicinal agent. Better in the open air, worse in a warm room. |
| | | | |
| | | |Case Pulsatilla pratensis - I used in the following case the 70,000th |
| | | |with success. M., aged forty-nine disposition, predisposed to lung |
| | | |affections, caught cold, and bronchitis set in, which soon ran into a |
| | | |chronic form. Various medicines were used unsuccessfully. After six |
| | | |weeks, the following symptoms were present and persistent: diarrhoea in|
| | | |the morning;cough moist and loose in the day, with copious |
| | | |expectoration of yellowish-green matter;cough dry and titillating at |
| | | |night when in a recumbent posture, preventing sleep. The patient |
| | | |imagined she had consumption, and was disposed to weep. |
| | | |Dr Hoyne. |
| | | | |
| | | |Thuja occidentalis-Thuja may be used when the attack commences in the |
| | | |afternoon, or after midnight. The face, during the paroxysm, is red, |
| | | |the urine too copious, and often passed involuntarily.Relief is |
| | | |afforded by throwing the head back and by cold drinks. Coryza after |
| | | |spells of asthma. |
