Questions on adult physical activity and exercise that have been asked on the National Health Interview Survey over the course of its history are shown here in their original format. The NHIS dates back to 1957, however, physical activity questions were first included in 1975. The full questionnaires from which these questions were extracted are available from the "Guide for Data Users" on the Adult Physical Activity Information website: .

The U.S .Public Health Service would like to know how many adults participate in some form of exercise, or in one or more popular sports.
